NARRATIVE
1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced annual inspection to the above facility on 02/05/2025. LPA arrived at the facility at 8:15AM, identified self and met with Elana Kornfeld, Licensee who guided analyst on a tour of the facility. LPA provided Licensee with a copy of the LIC 125 Entrance Checklist to help facilitate the inspection. This is a preschool program which consists of 5 classrooms. Facility operation hours are Monday to Thursday from 9:00 AM to 3:00 PM, and Friday from 9:00AM to 2:30PM. The Licensee also has on premises a facility under the name of Burbank Jewish Infant Center (197493886) that provides care to infants. All areas identified on the Facility Sketch were inspected. There were 28 children and 17 staff present when LPA arrived. The facility was observed to be within the license capacity and limitations. At approximately 9AM, LPA observed a male individual who was not associated to the facility present while children were being dropped off for care at the facility. Male individual was identified as Carlos Yanez. LPA observed Carlos Yanez assisting parents sign children in for care by presenting them with a QR code that gives parents access to the electronic sign in sheets. Per Licensee, Carlos Yanez is an armed guard and employee of Sire Protective Solutions who was hired May of 2024 per parent’s request, to provided security and protection during hours of operation. Per Licensee, Carlos Yanez has access to the facility should an emergency arise and needs to enter the facility where children are cared for. During record review, LPA did not observe any information indicating that the Department was notified of the armed guard at the facility. Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. ---Page 1 of 4

The following was observed during the tour of the facility:
Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. LPA observed cubbies in each classroom for children to store their belongings. LPA did not observe children belongs touching. The facility provides cots for children to nap. Parents provided clean linens for children on Mondays and facility sends home soiled lines home for washing every Friday. LPA observed napping equipment stored in a separate storage area. Age-appropriate sinks and toilets were inspected for availability and good repair in the restrooms. Classroom 1, Classroom 2 and Classroom 3 also has an additional sink for handwashing. General sanitation was observed. Availability of indoor drinking water was observed in classrooms.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Licensee has poisons are locked in a storage room. LPA was not able to verify that a carbon monoxide detector was operable. LPA contacted the local fire inspector who conducted the annual fire inspection. Per Fire Inspector, the fire alarm system has a built-in carbon monoxide detector that was tested and operable. Disaster drill log was available, last drill was conducted on 10/15/2024.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal.

Menus are posted one month in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. The facility provides am snack and mid-afternoon snack to children. Other meals are provided by parents or through the option of a paid meal service at the facility.
Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. There are outlets for drinking water accessible to children outdoors. LPA advised that no children shall be left without the supervision of a teacher at any time.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free. LPA did not observe pools or bodies of water at the facility. ---Page 2 of 4

Facility has conducted water lead testing to comply with Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018 requires all licensed Child Care Centers (CCC’s) constructed before January 1,2010 to test their drinking water for lead contamination between January 1, 2020 and January 1, 2023, and then every five years after the date of the first test.

Children’s Records were reviewed. Inspection of required forms was made and documented on the LIC 857.
LPA also reviewed staff records. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. The review of Staff records was documented on the LIC 859. Staff present had proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present also had proof against TB, measles, pertussis, and influenza.

Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in.

First Aid supplies were observed in every classroom inaccessible to children. According to Licensee, medication is only administered to a child when accompanied with a doctor's note and is stored in the office. The facility was observed to be equipped with an isolation area for any child who becomes ill and it is located in the office.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The following deficiencies listed on the attached on the 809-D page are being cited in accordance with California Code of Regulations Title 22.

LPA informed the licensee that this report documents 2- Type A citation(s) which shall be posted for 30 consecutive days as there are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA informed the licensee to provide a copy of this licensing report dated 02/05/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A Notice of Site Visit was provided. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
