25 | On 11/13/24 at 2:58 p.m. Licensing Program Analysts (LPA) Claudia Kam conducted a case management visit at the above facility for the purpose of follow up on facility plant update. Upon arrival, LPAs met with Carolina Carrasco, position assistant and provided LPA a tour of the facility. LPA observed proper care and supervision. There were 7 children in care 1 school age, 5 preschool age and 1 infant. Licensee arrived at 3:12 p.m. and provided tour of the construction being conducted
Facility is conducting construction to create an additional dwelling with a shared wall on the property. LPA has reviewed the construction plans and has confirmed that the city has approved the Additional dwelling and it has been assigned it's own address. LPA has observed fences on the property preventing access by the children to the construction area. At time of visit construction workers were observed on the property who do not have clearance.
Based on this information the following deficiencies on the LIC 809 D are being cited today 11/13/24.
LPA Claudia Kam informed facility representative Leonor Sanabria that this report dated 11/13/24 documents 1 Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Claudia Kam informed the facility representative to provide a copy of this licensing report dated 11/13/24 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
A violation regarding 102370(d) warrants an immediate civil penalty of $500 and is hereby assessed, see LIC 421IM or other form used.
A notice of site visit was given and must remain posted for 30 days.
