25 | On 01/30/2025 Licensing Program Analysts (LPAs) Mariah Aguirre and Monique Ayala conducted an
unannounced case management inspection. This inspection is to ensure the health and safety standards as required by the regulations governing child care centers are met. Inspection was conducted regarding an incident that came into the office on 12/20/2024. Report was submitted timely.
LPAs met with director Florence Banda who guided the analysts on a tour of the facility, indoors and out. A census was taken and staff names were documented. There were 61 children present with 11 staff, including director and cafeteria cook. LPAs discussed the purpose of the visit, to further look into the incident report submitted on 12/20/2024 with Ms. Florence Banda. LPAs attempted to conduct interviews with staff and children. Due to the lack of children associated with the incident being absent and unqualified for interview, LPAs were unable to conduct student interviews. Staff was not available to be interviewed as they were not present at the facility at the time of inspection. Based on the lack of information able to obtain on this date follow up is necessary regarding the incident report listed above.

A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
