Licensing Program Analysts (LPA) Cynthia Reyes conducted an announced Case Management inspection on this date for the purpose of to verify that the licensee received and has abided by the letter regarding, Sherry Keller: ORDER TO INDIVIDUAL/FACILITY OF IMMEDIATE EXCLUSION.

Upon arrival LPA met with Director Laura Perez and the nature of the inspection was discussed. LPA advised Director that as of April 26,29024, Sherry Keller is excluded from Kiddie Academy of Glendora (Infant 198016425/Preschool 198016423/School Age 198016424 Program) and any other facility licensed by Department of Social Services. Director was advised that the immediate exclusion letter is a public document. The Director was informed of the appeal process for this exclusion.

Director stated they did not receive the Exclusion Letter. LPA Reyes provided a copy of the letter and explained the letter to Director Laura on this date. Laura stated on 01/02/2024 that they sent a large list of disassociation's for all three components (Infant/Preschool/School Age). Laura stated they also Emailed LPA Reyes with the list and Laura also printed on this date the copies sent to the LPA on 01/02/2024. During todays inspection LPA reviewed and received the copies and a portion of the disassociation's list was cut off from the Infant list. The department disassociated Sherry Keller from the preschool and school age per the facility request. Laura also stated Sherry Keller has not worked for the facility since her last day working back on 11/14/2014. Sherry had only worked less then a month, due to her start date was 10/02/2014. Laura tried disassociating Sherry now, but guardian is not working at this time for her and will submit the disassociation by tomorrow morning

Exit Interview conducted with Director, Laura Perez. Notice of Site Visit (LIC 9213) posted and – must remain posted for 30 consecutive days. Failure to maintain posting as required will result in a civil penalty of $100.00. A copy of Appeal Rights was explained and provided.
