Licensing Program Analyst (LPA) Jonnisha Culbert conducted an unannounced annual/random inspection at the facility noted above. LPA met with Director, Vanessa Zylstra discussed the purposes of the visit and provided them with Entrance checklist for Child Care Centers (LIC 125). Director guided analyst on a tour of the facility. This is a preschool center located on the premises of Generations Family Church. The preschool consists of seven classrooms. Per Director, currently, only two classrooms are utilized. The program operates from Monday through Friday from 7:00am to 6:00pm. The program offers care to children 2 to 5 1/2 years old. Present during today's inspection were Director, 3 staff, and 12 children.

Upon arrival, LPA observed all required postings in a prominent publicly accessible area.

All areas used by children were inspected for safety. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. The facility uses central heating and air for ventilation. Lighting was observed to be in operable condition. LPA observed age-appropriate toys. Children have their own cubby in each classroom to store their belongings. Cots are available to children for napping. Per Director, when children become ill, they are isolated and remain in the office until their authorized representative arrives to pick them up.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Disinfectants and cleaning solutions are kept in cabinets inside of the Teacher’s lounge. In the kitchen, medication is kept inside of a box inside. LPA observed that medication inside of the box were labeled with child’s name. LPA reminded Director that medications must be in its original packaging. Poisons were not observed during visit. Per Director, there are no poison on the premises. LPA reminded Director that if poisons are purchased, they are to remain in an area that is locked and inaccessible to children. A carbon monoxide detector was observed and is operable. Fully charged fire extinguishers are available if needed and there are fire pull stations located by every door.

Based on record review, Director did not comply with the section cited above. LPA reviewed waterboard website and did not find record of lead test performed for facility. This is a potential health and safety risk to persons in care.

Per Director, they can not perform lead testing because the facility is closing on April 10, 2025. Director provided LPA with signed license, an explanation for closure, and letter that was sent to parents to inform them of closure.

The outdoor playground area was evaluated. LPA observed all areas around and under high climbing equipment and similar equipment are cushioned with mulch. There is adequate shade on the playground. Per Director, there are no pools or bodies of water at the facility. Availability of outdoor and indoor drinking water was observed in classrooms via water bottles. Per Director, teachers assist children with retrieving water from water dispenser.

LPA reviewed waterboard website for facility lead exceedance results. Per Director, they do not know if the facility was tested for lead contamination. LPA observed that a lead test was not conducted on the facilities’ water. This is a potential health and safety risk to the children in care. Per Director, the children only use facility water to wash their hands. LPA informed Director that they are required to provide the Department with a Lead Exceedance Clearance or obtain a waiver from the Department. LPA provided Director with Provider Information Notification (PIN) 21-21.1-CCP for guidance.

Children and staff records were reviewed for completeness. LPA observed that 2 out 2 children records were complete and 4 out of 4 staff records were complete. All staff met educational requirements. LPA observed that everyone had mandated reporter certificates, and immunization records. 2 out of 2 staff had cardiopulmonary and resuscitation and first aid training certifications. Children's roster was reviewed and is current. LPA reviewed sign in and sign out app and observed that parents used electronic signature to sign children in and out.

Disaster drills were logged and available. The last drill was conducted on 1/13/2025. Menus are posted a month in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. LPA inspected the food preparation area. The kitchen was observed to be clean and free of litter. Food supplies were observed to be in good condition. The facility provides two snacks and parents provide lunch.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA referred director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

During visit one type B citation was issued. Appeal rights were given to Director. Appeal rights and notice of site visit was given and must remain posted for 30 days.
