On June 10, 2026, at 10:45 am Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced annual inspection at the above facility. LPA was greeted by licensee, Alana Lewis who guided LPA on a tour of the facility. License’s assistant was also present during the inspection. All adults present during the inspection have criminal record clearance. Per licensee there are 9 children enrolled and LPA observed 7 children in care. Hours of operation are Monday through Friday from 7:00 am to 5:00 pm. LPA provided a copy of Entrance Checklist—Family Childcare Home (LIC126) to licensee.

Family members residing in the home is 1 adult who obtains criminal record clearance. This is a single-story home that consists of 2 bedrooms, 1 bathroom, kitchen, living room, laundry room, front yard, backyard (fenced) and ADU (playroom).

Off limit areas include 2 bedroom, kitchen, laundry room and front yard.

Areas accessible to children are the bathroom, living room, ADU (playroom), and backyard which is fenced.

At 11:00 am, licensee guided LPA on a tour of the areas that are accessible to the children in care. Upon entrance from the front door, LPA observed the living room. LPA observe a fireplace in the living room, which was barricaded and made inaccessible to children in care. LPA also observed napping equipment in the form of cots and mats for children in care. Bedding was inspected for appropriate storage and cleanliness. Per licensee all bedding is washed at the facility weekly. LPA then inspected bedroom 1 and observed clothes and other belongings of the licensee. Previously, bedroom 1 was accessible to children in care. Per licensee bedroom 1 is no longer accessible to children in care and will provide an updated facility sketch. LPA then inspected the bathroom which is next to bedroom 1 across the living room area.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The bathroom was inspected and clean and orderly. LPA observed cleaning solutions in a cabinet under the sink. LPA observed a child safety lock to be attached to the cabinet; however it was broken. Per licensee they will replace the child safety lock to ensure the children do not have access to materials under the sink. LPA then inspected the backyard which is fenced and observed more age-appropriate toys for children in care. Then the ADU (playroom) was inspected where several age-appropriate toys were observed along will parent board. All areas were observed to be clean, orderly, comfortable, and well ventilated.

There are first aid supplies available. Per licensee, the facility provides all meals to children in care. LPA reminded licensee, if a child brings their own food, their containers must be labeled with their name, stored properly and or refrigerated.

If a child becomes ill, the child is isolated in the living room while the child is picked up. A cot is available if needed.

Licensee's poisons, detergent, cleaning compounds, medications and other items which could pose a danger to child are stored where they are inaccessible to children.

LPA observed a functional smoke and carbon monoxide detector in the playroom and living room area. LPA observed a 2A-10BC fire extinguisher. The valve on the fire extinguisher pointed to the fully charged green section. Per licensee, the fire extinguisher was serviced on 04/21/26. There is a working telephone on site. Per Title 22 Emergency Disaster Drill must be conducted and documented at least every 6 months. Per licensee, the last emergency disaster drill was conducted and documented on 3/10/2026.

Per the licensee, there are no firearms, bodies of water or pets on the premises. Licensee obtained current Pediatric First Aid and CPR certification expires on 4/28/2028.

LPA conducted a children file review and deemed not complete. 2 out of 4 files did not have LIC995A Parent Rights. 2 out of 4 files did not have a 15 – minute safe sleep log available. LPA provided licensee a safe sleep log to used and document what the regulation requires. LPA also advised licensee to go over safe sleep regulation and safe sleep PIN. 2 out of 4 files did not have proof of their immunization record. 1 out of 4 files obtain prescribed medication at the facility and do not obtain a form where the child’s representative is explaining how to administer the medication or granting the licensee permission to administer the medication.

LPA explained to the licensee that it is best practice to develop an Incidental Medical Plan as they are administering prescribed medication to children in care. The facility will be given a technical advisory and provided a copy of PIN 22-02-CCP.

LPA conducted staff file review and deemed not complete. 2 out of 2 staff members did not provide proof of measles immunization, 1 out of 2 staff members did not provide proof of tuberculosis test (TB), 1 out of 2 staff members did not provide proof of employee rights from LIC (9052), 1 out of 2 staff did not provide proof of childcare mandated reporter certificate.

Licensee completed Mandated Reporter training and expires on 12/28/2026. All childcare employees must complete mandated reporter training within 30 days of hire and renew training every two years at www.mandatedreporterca.com

Licensee has the required documents posted in the FCCH; Facility License (LIC 203), Notification of Parents' Rights Poster (PUB 394) and Emergency Disaster Plan (LIC610a) to be posted.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Based on the LPAs observations and record review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee, Alana Lewis.
