32 | The home maintains telephone service via land line and cell phone. The home is observed to be clean and orderly. There are toys and other age appropriate material available for children. LPA observed that detergents, cleaning compounds and medication are inaccessible to children. Licensee understands that all poisons must be lock, not only inaccessible to children. Isolation area for sick children waiting to be picked up is in the living room, away from the other children. Per Licensee, there are no firearms or weapons stored in the home. At this time, the Licensee does not have any children who stay overnight. There are no pools or spas, or other bodies of water.
Currently Licensee cares for one infant. LPA observed one play yards visible in the living room. Napping equipment does not block entrances or exits. Infant mattresses were observed to be firm with tightly fitted sheets. LPA did not observe loose object, bumpers, objects hanging, or objects attached to the play yard. LPA reminded Licensee of the new Safe sleep regulations, Infant Sleep Plan for infants under 12 months and 15 minute sleep check documentation for infants 0-24 months. Licensee stated that she was unaware of the LIC 9227, LPA printed a form for Licensee.
Currently, due to weather children are not playing outside. LPA observed a shared courtyard where children play. Licensee also stated that sometimes they go on short walks around the neighborhood. Licensee was reminded that all children need to be fully supervised when children are outside.
Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, and documentation of 15 minute Infant Sleep Check (0-24 months).
Staff records were reviewed for approved Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse.
At 10:40am, Licensee provided LPA with a current CPR & 1st Aid, however it does not show it is EMSA approved. LPA tried multiple times to verify but the number for the training services was phone line was disconnected. LPA then called EMSA, and operator stated he will call back. Type B was cited.
