25 | Licensing Program Analysts (LPA) A. Padilla and T. Tran conducted an unannounced Case Management-Incident inspection. This inspection is regarding an incident that was received by MPSW Licensing department on March 20, 2024 regarding a parent concern of personal rights of children. LPA met with Site Supervisor Maria Labrada who assisted with the inspection.
LPA conducted interviews with three staff, child involved in the incident, and other. Staff indicated that they did not witness anything unusual involving the child on the date of the incident. Based on record review 17 children enrolled with 4 staff members present. Staff indicated that they followed up with parents regarding incident and that there are no disconnections and concerns with staff. Child involved in incident continues to attend regularly. Per site supervisor the facility had no concerns with staff interactions with child involved during incident.
Based on today inspection 4/9/24 there was no violations found of children personal rights violated. There are no deficiency cited during todays inspection.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
