Licensing Program Analyst (LPA) Seung Lee conducted an unannounced case management inspection. Upon arrival LPA Lee met with Director Christine San-Cobb. Along with the Director, Child development and family specialist Dr. Patricia Thomas and Assistant Head of School Elizabeth Desmarais

The purpose of the inspection conducted today was to obtain additional information regarding an incident that was reported to the child care regional office on 02/22/2024. During the course of this inspection, LPA Lee conducted interviews, reviewed documents, and made observations in regards to the incident.

On 02/21/2024 around 11:30AM Staff#1 was in the teacher's break room with Child#1. There was a cup of coffee that was resting in the table of the break room that was just made by Staff#1. Child#1 had small container of coffee creamer in their hand and was trying to pour the creamer on the coffee that was sitting at the table. During this time Child#1 had accidentally nudged the coffee with their elbow which caused the coffee in the cup to spill on Child#1's arm near the elbow area. Staff#1 was present in the room with Child#1 during this time and witnessed the entire incident. Child#1 was immediately provided first aide from Staff#1. The Director and a Nurse that works for the facility were informed of the incident. The parent of Child#1 was also informed of the incident by facility staff over the phone. Around 1pm, Child#1 was picked up by a parent and taken to the Doctor. Child#1 returned to school the next day on 02/22/2024.

During the inspection, LPA Lee observed that the break room where the incident occurred was the break area for Staff members. Based on the information obtained during a prior inspection, LPA Lee asked the facility if Children in care are allowed in the break room for any reason. The facility confirmed that the break room for staff members is supposed to be off limits to children in care. During an interview, Staff#1 stated that they had decided to take Child#1 with them to the break room in order to calm the child down before having their lunch. Staff#1 stated that Child#1 wanted to help with making the coffee which is why the child had the creamer in their hand before the incident had occurred.

While it is understandable that a Staff member would find the need to redirect a child away from the other children in a classroom, this could have been done without taking the child to an off limits area of the facility. While a child in care might want to do things like pour cream into a cup of coffee, partaking in this type of activity even with staff present is an immediate risk to children in care. Even if this incident had occurred in a classroom where children are allowed to be present, a cup of any beverage that is hot should not be accessible to a child in care.

Based on the information obtained and confirmed during this inspection, the following deficiency listed on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. A copy of the LIC 9224 was provided to the facility during this inspection. The Director understands that a signature on the copies of this form is to be filed for every child currently attending and any new children enrolled for a period of 1 year starting from this date.

The notice of site inspection along with all pages of this report must remain posted for 30 days during hours of operation. Failure to maintain posting during business hours will result in a civil penalty of $100.00 dollars.
