Licensing Program Analyst conducted an unannounced complaint inspection at the facility. LPA met with Site Supervisor Jessica Osorio and explained the reason for the visit. There were children supervised by two teachers during the visit.
During this investigation LPA conducted interviews with staff, reviewed records and obtained pertinent documents. Staff interviews revealed that on August 7th 2025, a child exited the facility via the outdoor gate. Per Staff 1, they observed Child 1 hit Child 2 and as they were walking over to Child 1 to address the incident, Child 1 ran out of the gate door. During today's inspection, LPA observed that the gate door is easily accessed by a pushing mechanism. Staff 2, corroborated that Child 1 exited the facility and that Staff 1 was able to run after them immediately and bring them back to the facility. Per staff interviewed, the children were being properly supervised and a plan of action for Child 1 was implemented. LPA confirmed that there were two teachers and 11 children present during the incident.

LPA advised for the gate door to be made inaccessible to children in care to prevent additional incidents.

Based on interviews, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.

A notice of site visit was given and must remain posted for 30 days.
Exit interview conducted and a copy of the report was provided to Site Supervisor Jessica Osorio.
