Licensing Program Analysts (LPAs) Jeanette Estrada and Tyler Reyes conducted a case management inspection at the facility for the purpose of following up on an unusual incident report (UIR) submitted to the Department on 2/13/25. LPAs initially met with Facility Representative Maria Vallin. LPAs advised Facility Representative of the reason for the visit. Facility Representative guided LPAs on a tour of the facility. LPAs observed 16 children with two staff in classroom 36, 15 children with three staff in Classroom 37 and 16 children with two staff in classroom 38 and 11 children with three staff in Classroom 39. Early Learning Center Managers, Monica Alvarado and Maricela Guido joined at a later time.

It was reported on 2/13/25 that on 2/12/25, Child 1 sustained an injury for which paramedics arrived to assist. Per Staff interviewed, Child 1 was playing during free choice, Child 2 called for teachers when they observed Child 1 laying on the floor next to the cubbies. Per Staff 1 and Staff 2, Child 1 was observed laying in a fetal position with their arms to their chest while the right side of their head was on the edge of the cubby. Per Staff interviews, they immediately attended to Child 1. Per Staff 1, they observed Child 1 was unresponsive and immediately notified the site's Head Teacher. Staff 1 stated they also attempted to notify the Elementary School nurse but they were not immediately available therefore they decided to contact 911 which arrived within 5 minutes.
Per Staff 2, they attended to Child 1 and were trying to get Child 1 to respond. It took Child 1 about 30 seconds to respond and per Staff, it took Child 1 a couple minutes to fully come to.
Per Staff, Child 1's parent was contacted upon the occurrence of the incident. Parent arrived a few minutes after the paramedics arrived. Parent left the facility with Child 1 and Child 1 was taken to the hospital. Per Staff, Child 1 returned to the facility the following week with no medical restrictions.

Based on interviews and record review, the facility responded to the incident by attending to the child and notifying the appropriate parties upon the occurrence of the incident. No deficiencies are cited today.
Exit interview was conducted with Monica Alvarado and Maricela Guido, Early Learning Center Managers. A copy of the report and the Notice of Site Visit was provided.
