On 08/15/2024 at 11:45 AM an unannounced Annual/Random Inspection was conducted by Licensing Program Analysts (LPAs) Katrina Chicote and Ashley Calderon. LPAs announced purpose of visit and initially met with Facility Representative, Yvonne Vasquez, Principal. Facility is located in Classroom 36 on Wisdom Elementary School Campus and currently licensed for a capacity of 24. Per FR, they currently have six children enrolled. The program currently operates Monday thru Friday from 8:00 AM to 2:30 PM. LPAs singularly toured the facility of both indoors and outdoors. Required postings were discussed. Per File Review, facility has two waivers for use of shared Outdoor Play and Bathroom with elementary school campus, but did not observe waivers posted at time of inspection. LIC 125 was provided.

The facility is made up of one classroom. At the initial start of the site visit, there is a total census of five children and a total census of two staff (S1 and S2) present. Per FR, there is currently a long term substitute in place. Sign-in and out sheets and procedures were reviewed. LPAs observed all children signed in at time of inspection.

Beginning at 11:50 AM, furniture and equipment were inspected for age appropriateness and good repair. LPAs observed surface of classrooms to be safe and clean, and child size tables and chairs available for children with rounded edges and well maintained. LPAs observed learning stations for dramatic play, math, and science with a variety of age appropriate toys maintained free of loose and sharp parts and a library area available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Cots were inspected. Per Staff, linens are provided by the facility and washed by weekly laundry service. Availability of drinking water was reviewed. Per S2, children bring in personal water bottles and are refilled through drinking fountains. LPAs observed children's personal water bottles in their cubbies. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPAs observed bathroom available in classroom that had two toilets and two sinks, a third sink available outside of the bathroom. Hazardous items including poisonous cleaning compounds were not stored inaccessible to children at time of inspection. LPAs observed these items stored on low counter by child's sink. A first aid kit is kept in the classroom and in school's office. Per FR and Plant Maintenance, carbon monoxide detectors are hard wired into smoke alarm system. Plant Maintenance, pointed out carbon monoxide and observed a green light indicating operable at time of inspection. LPAs observed smoke detectors and fire extinguishers are present in the facility. LPAs discussed requirement to have fire extinguishers serviced annually and to conduct regular checks of smoke and carbon monoxide detectors to ensure they are in operable condition. Disaster drill log was available and last drill conducted was on 08/12/2024. Isolation area for sick children is in school's Nurse's Office. Per FR, they currently have no children that require medicines.

Snack/lunch menus were reviewed and observed posted. Facility provides AM Snack, Lunch, and PM Snack. Per FR, food is delivered from the elementary school campus. LPAs observed trash cans being used to discard food do not have a tight fitting lid inside classroom and outdoor play area.

Beginning at 12:10 PM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPAs observed surface of outdoor play areas to be safe and maintained for activities planned such as concrete for bike paths. LPAs observed multi-level age appropriate play structure that has cushioning material below to absorb fall that is well maintained at time of inspection. Required shade and fencing were inspected. LPAs observed bench with shade and two pop up tens available for shaded areas for children. LPAs advised FR that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to drinking fountains during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Personal Rights of children were observed by LPAs. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District (LAUSD).

Beginning at 1:20 PM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. LPAs did not review confidential staff files during the inspection as they are stored at the administrative office. CPR card expires on 06/2025 for S1. Staff present did have proof of the AB1207 Mandated Reporter Training certificate on file. Report Continues - Page 2 of 4

The following were discussed:
LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation, this includes any changes as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. Please complete the survey and share your inspection experience. Licensee (or facility representative) was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

CCC COMPLETED TESTING AND IS IN THE PROCESS OF REMEDIATING LEAD EXCEEDANCES:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

Incidental Medical Services (IMS) - Child Care Centers
Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. MyChildCarePlan.org – Centers and Family Child Care Homes Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Based on this information the following deficiencies on the LIC 809 D are being cited today, 08/15/2024.

LPA Chicote informed Facility Representative Yvonne Vasquez that this report dated 08/15/2024 document(s) one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.



Also, LPA Chicote informed the Facility Representative to provide a copy of this licensing report dated 08/15/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification

A notice of site visit was given and must remain posted for 30 days.
