Licensing Program Analyst (LPA) Seung Lee conducted an unannounced plan of correction inspection. Upon arrival LPA Lee met with site Director Yin Fang Hai

On 08/10/2023, this facility was cited after it was observed that lead testing was completed in June 2021 but not in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

During the inspection LPA Lee was provided a copy of the of water sample results which was completed on 06/17/2021.

The notice of site inspection must remain posted for a period of 30 days during hours operation. Failure to maintain posting will result in a civil penalty of $100.00 dollars.

Exit interview conducted with Director Yin Fang Hai . Appeal rights discussed and explained.
