25 | REQUIRED- 1 YEAR INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced Required- 1 year inspection to the above facility on 09/23/2022. LPA arrived at the facility at 11:39 PM and met with Marieta Tahmazyan, Licensee who guided analyst on a tour of the facility. Also present during this inspection, were Arevik Arakelyan and Mariyana Asatryan, Licensee’s Assistants. Per licensee, Marieta Tahmazyan facility hours are Monday to Friday from 7:00 a.m. to 10:00 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee.
This is a one story home which consists of 3 bedrooms, 1 and 1/2 bathrooms, kitchen, dining room, living room , detached converted garage which consist of a room and half bathroom, storage in the backyard, front yard and backyard (fenced). Per licensee at 12:00 p.m. the children use the last bedroom (main daycare area), half bathroom in the daycare area, and back of backyard (fenced). Per licensee areas off limits to children and parents include: 2 bedrooms, living room, dining room, kitchen, one bathroom, detached garage, half bathroom inside the garage, storage, front yard, and front part of back yard. LPA did not observe any stairs in the home. Per licensee, parents enter the home through the side gate which leads to the daycare room. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 12:10 p.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the last bedroom (main daycare area). LPA observed 7 children were playing in daycare area with licensee and her assistants. LPA observed children tables, chairs, toys and napping cots in daycare area. Next LPA toured the half daycare bathroom in daycare area and did not observe any hazards.
