25 | Licensing Program Analysts (LPAs) T. Tran and P. Bowden arrived at the above facility to conduct a Case Management Incident inspection that was self-reported occurred on 11/4/24. The Monterey Park Southwest Office received the writing report on 11/05/24. During the inspection, LPAs observed proper care and supervision.
During today's visit, LPAs completed child and staff’s files review. LPAs obtained personnel report, child, and facility document. LPAs conducted interviews with children, staff and other. Based on the available information obtained through the course of the interviews. On the day of the incident, there was a staff supervised four children. S1 (see LIC811) observed a child pulled C1's shirt while playing outside in the yard causing C1 to fall on the ground. S1 inspected C1's arm and no visible indication that child was hurting. On 11/4/24, parent contacted the school to report C1 had a fractured right elbow and was taking to the hospital to seek medical care. Although the incident happened, based on the Title 22 regulation, this incident does not appear to be a violation for care and supervision. No deficiency was found during today's inspection.
