An unannounced Required Annual site inspection was conducted by Licensing Program Analyst (LPA), Bardo Baluyot, who met with Teacher,Crystal Luna. This facility operates both a full day program (2 classrooms) and Head Start program (4 classrooms) which has an AM and PM session. Head Start classroom #1 had already left for the day during the time of inspection.

LPA observed a total of 12 staff with 28 children. LPA found that facility is within ratio and care and supervision of children is being met. LPA observed during this inspection that facility is in compliance of this. LPA reviewed sign in/outs sheets and children's rosters for each classroom.

Rooms identified on facility sketch were inspected. Furniture and equipment were inspected for age appropriateness and its condition. Telephone service, heating, lighting and ventilation were evaluated. Storage space/area for children's belongings were inspected. The condition and age appropriateness of children’s sinks and toilets were inspected. LPA evaluated general sanitation and cleanliness of facility. Availability of indoor drinking water was observed in form of water jugs with disposable cups.

Outdoor area and equipment was inspected for safety, cushioning material, condition and age appropriateness. Required shade, drinking water (water jug and disposable cups) and fencing were inspected. Play area was inspected for hazards and inaccessibility to bodies of water.

LPA inspected First Aid kit’s supplies and completeness. Smoke and Carbon Monoxide detectors are hardwired into facility on the ceilings and are functional. Per Ms.Luna, they are inspected by the Fire Department annually.

LPA inspected the kitchen on site where there was one staff member. Kitchen provides meals and snacks for both the full day and head start programs. Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation area was inspected. A review of cleaning and food supply storage areas was made.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. No children or staff files were reviewed during this visit as they are kept in the main office: Head Start staff and children's files are stored at the Options Head Start main office, located at: 404 Woodbury Road, Altadena, CA 91001 (626)204-8900 and CDC staff and children's files are stored at the Options Early Learning Center’s main office, located at: 885 S. Village Oaks Drive. Covina, CA 91724.

After a complete inspection of the facility, there were no deficiencies observed on this date according to California Code of Regulations Title 22 Division 12.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
