An unannounced Annual Required Inspection was conducted on 09/17/2021 at 12:00 PM by Licensing Program Analyst (LPA) Katrina Chicote. Facility is currently licensed for a capacity of 36. LPA Chicote conducted COVID-19 screening call prior to entering facility. Licensing staff met with Sylvia Aguilar, Director. The program currently operates Monday thru Friday from 7:30 AM to 4:00 PM. LPA was taken on a guided tour of the facility of both indoors and outdoors.

At the initial start of the site visit, there is a total census of 18 children and a total census of five teachers present. The facility is made up of three classrooms, Preschool (Rm. 114), Older Toddler (Rm. 111), and Young Toddler (Rm. 108). Furniture and equipment were inspected for age appropriateness and good repair. LPA observed Lakeshore wooden shelves filled with puzzles, blocks, science toys, soft dolls, wooden play kitchen, and other child appropriate toys in each classroom to be clean and free of hazards. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings in each classroom. Mats/cots were inspected, LPA observed mats/cots stored in facility's kitchen. Per Director, linens are provided by the facility and washed weekly or more often if needed. Availability of drinking water was reviewed. LPA observed water faucets in each classroom, per Director water faucets provide filtered water for drinking water in each classroom. Preschool (Rm. 114) has own bathroom and Older Toddler (Rm. 111) and Young Toddler (Rm. 108) share bathroom. LPA observed age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation.

A first aid kit is kept in cabinet of each classroom. Carbon monoxide detectors and smoke detectors are present in the facility. Fire extinguishers have been serviced, service tag was observed. Hazardous items including poisonous cleaning compounds were stored inaccessible area to children.

Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness and proper equipment. Facility serves breakfast lunch, and PM snack. Food is provided by food program partnership with the hospital.

Outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. Required shade and fencing were inspected. LPA observed large play structure with steps and slide with soft cushioning material below. Per Director, this play structure is currently not being used as a precaution for COVID-19. LPA observed outdoor play area to have sand box area, child appropriate tables and chairs, art materials, and other age appropriate toys free of lose and sharp parts available for children in care. Per Director, they also use the basketball court directly next to this play area Mon-Thurs for bicycles and other gross motor play and they also have access to park across the street with parents permission. LPA Chicote advised Director that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to drinking fountains during outside play. LPA observed water fountains and Director states teachers bring out water jugs with cups outside and to the park. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. Personal Rights of children were observed by LPA.

Staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for adults. Inspection of required forms made.

Incidental Medical Services:
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following were discussed with the Director:
The Director was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.

LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.

LPA informed Director to log onto web site www.ccld.ca.gov to obtain forms and LIVESCAN application. Records for all children and staff must be maintained for three (3) years after separation from the facility.

The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 09/17/2021.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
