On 05/10/24 at 11:30 am Licensing Program Analysts (LPAs), Claudia Kam made an unannounced required visit at above licensed facility. LPA disclosed the purpose of the inspection and was granted entry into the facility.

LPA met with Veronica Medrano and was taken on a guided tour of both indoors and outdoors of the facility. LPA observed Teacher-child ratios, and staff names recorded. All center staff that was present during today’s inspection had fingerprint clearance and are associated to the designated license number. Care and supervision evaluated it is determined that the basic needs and personal rights of children are met and appropriate. Sign-in and Sign-out sheets were reviewed and found to be in compliance.

The hours of operation are 7:30 am – 5:00 pm, Monday - Friday. Facility is currently licensed for a capacity of 36. At the initial start of the site visit, there is a total census of 26 children and a total census of 8 teachers present. The facility is made up of 3 classrooms. Room 108: had 7 children and 3 staff, Room 111: had 5 children and 2 staff, Room 114: 16 children and 3 staff at time of visit. LPAs observed all posting requirements for operation on the posting board and the following documents were observed LIC 203A-License, LIC 610 A-Emergency Disaster Plan, LIC 9148-Earthquake Preparedness Check List, PUB 394-Notification of parent’s rights poster, LIC 613A- Personal Rights, PUB 269- Child Car Seat Law. Menus and activity schedule are posted in each classroom for parents

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Children napping equipment met the requirement regulation at time of inspection. Storage for children's belongings, bathrooms facilities, and isolation areas were inspected. Age-appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, safety, and sanitation. LPA observed cubbies with wooden toys, puzzles, books, and other appropriate toys to be clean and free of hazards at time of inspection. LPA observed carpets and age-appropriate tables and chairs in each classroom. LPA observed backpacks for first aid kept in each classroom. First Aid supplies were inventoried and were found to be in compliance with regulation.

Breakfast, snack and lunch menus were reviewed at time of inspection. Facility provides breakfast, lunch, and snacks. All food items delivered daily and prepared by the hospital California Hospital Medical Center. Food allergies are shared with the vendor to ensure that meals are prepared to be allergen free. Drinking water is provided via water cups. Carbon monoxide detectors and smoke detectors are present in the facility. LPAs tested detectors at time of inspection and were determined to be in operable condition. Fire extinguishers were observed as serviced on 4/19/24. Hazardous items including poisonous cleaning compounds were stored inaccessible area to children. A review of medication policy, including administering, labeling, storage, and records was made. Fire & Earthquake drill observed conducted monthly with last drill conducted on 4/24/2024 per observation of log.

At 11:30 AM, LPAs observed outdoor play area which consists of a large play structure that is shaded and has cushioning material covering the ground below and perimeter. Outdoor equipment was inspected for safety, cushioning material, good repair, and age-appropriateness. LPAs observed sandbox area, sensory tables, a play structure with a plastic slide and climbing wall, and other age-appropriate toys around the play yard. Required shade and fencing were inspected. There is a community shared space that is used for games and tricycles that connected to the playground. The door is locked and is inaccessible when not in use. A waiver is posted detailing the approval and scheduled times for usage. LPAs advised Director that the children need to be always within the direct care and visual supervision by the teachers. Per Director, the children have access to drinking water via the sink and disposable water cups. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection.

LPAs reviewed with Veronica Medranos the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for adults. LPA observed current CPR and First Aid Certification for center staff. CPR card expires on 06/2024 for Staff Member. Mandatory Forms for the children’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Director was advised how to access forms and Regulations online at www.ccld.ca.gov. to ensure the most current information regarding regulations for all those who assists in providing care.

The following were discussed with the Facility Representative: The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked, and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.



Center was reviewed for lead compliance and center meets standards for lead. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Medication was found to be properly labeled and current with an expiration of 6/24/2024 for epinephrine for allergy interactions. Facility representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview the facility representative confirmed that there are no registered sex offenders living in the facility and LPA completed the RSO profile in FAS.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 5/10/2024.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative.



On 05/10/2024 at 1:30 PM, exit interview was conducted with Childcare Manager, Veronica Medrano. A copy of this report was given, and appeal rights were issued and discussed.
