Licensing Program Analyst (LPA) Katrina Chicote conducted an unannounced Annual Required inspection on 09/17/2021 at 9:50 AM. LPA Chicote conducted COVID-19 screening call prior to entry. Upon entry, LPA met with Sylvia Aguilar, Director, who guided analyst on a tour of the facility. This is an infant program which consists of two classrooms, Young Infant (Rm 107A) and Older Infant (Rm 117A). A Preschool component, facility #198017686, shares the space and each program has a separate entrance. Facility operation hours are Monday to Friday from 7:30 AM to 4:00 PM.

All areas identified on this report were inspected. Upon arrival, there is a total census of six infants present and a total census of six teachers present. Young Infant Room: three children and three staff. Older Infant: three childern and three staff. Teacher-Child ratio was observed to be in accordance with Title 22 Regulations. All children were observed to be under visual supervision of a teacher at all times. Classrooms were observed to ensure that infants are never left unattended and under the direct visual supervision of a staff person at all times.



The following was observed during the tour of the facility:
Children's roster was reviewed and is current. Sign-in and out procedures were reviewed. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Children present were signed in. Disaster drill log was available, last drill was conducted on
09/17/2021. LPA observed required licensing documents posted on bulletin board in the main entrance to the facility.

Indoor space is physically separate from other child care components. Furniture and equipment were inspected for age appropriateness and good repair. Toys were observed to be safe, and do not have sharp points, edges,

splinters and are not made of small parts that can pose a choking hazard. LPA observed wooden Lakeshore shelves filled with soft blocks, large manipulative toys, balls, puzzles, and wooden play kitchen in each classroom. LPA did not observe any baby walkers or any items that fall into that category present in the facility. All materials and surfaces accessible to children are toxic free. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. LPA observed carpets and soft play mats on the floor of each classroom and floor to be free of hazards.

LPA observed children have their own cubby to store their belongings in each classroom. Napping equipment was observed in a separate room. Storage for infants’ belongings and napping equipment/cribs was inspected and meet all regulatory requirements. At 10:00 AM, LPA observed standard blue cots for used in each classroom. Per Director, they do not have infants younger than 12 months at this time, standard Community Playthings cribs are kept in a storage room at this time. Per Director 1 teacher is always supervising the nap room while infants sleep. (LPA discussed Safe Sleep Practices and form provided)Linens are changed daily or more frequently, as needed by staff. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. Young Infant and Older Infant Room share a bathroom that is located in between the classrooms. General sanitation was observed. At 10:12 AM, LPA observed that infant changing tables have a padded surface no less than one-inch-thick and are covered with washable vinyl or plastic. The infant changing tables have raised sides that are at least 3 inches high. LPA observed that infant changing tables are within an arm’s reach of a sink.

Telephone service, heating, lighting and ventilation were evaluated and are in operating condition. Per Director, the isolation area is located in the her office. LPA observed a blue mat in the isolation area for ill children. Parents are contacted immediately when children are determined to be ill. Availability of drinking water was reviewed, per Director faucets provide filtered water that teachers use for drinking water. First Aid supplies were observed in each classroom.


Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were observed to be inaccessible. According to the Director, medication is only administered to a child when accompanied with a doctor's note and is stored in each classroom. LPA observed a locked medicine bin for use for medications in each classroom. Director states at this time they have no children requiring medication. Director states that there are no poisons stored at the facility and understands

that storage areas for poisons must be locked with a key or combination lock. Facility has one or more functioning carbon monoxide and smoke detectors that meet statutory requirements.

At 10:19 AM, LPA observed kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. Meals are provided by food program partnered with the hospital. Menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides AM snack, lunch and PM snack for children who eat table food (per needs and service plan). All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. All foods/beverages are stored in covered containers at 45˚ (F) or less. LPA observed refrigerator log and refrigerator temperature at 31 degrees.

Outdoor space is physically separate from other child care components. Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. At 10:20 AM, LPA observed outdoor space to have child appropriate tables and chairs, plastic play kitchen, small play structure with slide and a maintained garden. Due to COVID-19 precautionary measures, outdoor space is divided between two infant classrooms by a wooden divider, keeping cohort groups separate when playing outside. LPA observed outdoor activity space surface to have soft cushioning material that is maintained in a safe condition and is free of hazards. Areas around and/or under climbing equipment and slides have cushioning material to absorb a fall. The outdoor area has adequate shade. The Director states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this visit. Director states there are no weapons or firearms on the premises and none observed.

All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Staff Records were reviewed to ensure that a health screening report is on file. Infant Teachers files were reviewed to ensure that teachers present are qualified. Children’s Records were reviewed to ensure that Identification and Emergency form and a Needs and Services Plan are on file.

Incidental Medical Services (IMS) Policy:
This facility provides Incidental Medical Services – No children currently on medication. IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA advised the Director to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 09/17/2021.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
