On 3/18/25 at 2:55pm, Licensing Program Analyst (LPA) Ashley Calderon conducted an unannounced annual/random inspection. LPA Calderon knocked on door, rung door bell and called Licensee Charisse McDonald. McDonald arrived at the facility at 3:10pm with staff assistant Laura Caranza. LPA disclosed the purpose of today's visit and was granted entrance to the facility by Licensee Charisee McDonald.

Upon entrance LPA Calderon observed 4 children in care and 1 staff assistant (staff is fully qualified, record reviewed). All adult individual's present at the home are fingerprinted cleared and associated . Facility capacity is in compliance for a Large Family Child Care Home. Hours of operation are Monday-Friday 7:00 AM - 5:00PM.

This is a one-story home which consists of 4 bedrooms, 2 bathrooms, kitchen, dining room, living room, front yard and backyard. The children use the bathroom in the hallway next to the den. LPA observed that there are no fireplaces at the facility. Per Licensee, areas off limits to children and parents include: all 4 bedrooms, living room, dining room, 1 bathroom, kitchen, and the back yard. The only rooms accessible to the children is the den (kid room), 1 restroom, and the side yard.

LPA conducted tour of the facility along side with C.McDonald. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. The following was observed and reviewed during this inspection:

LPA reviewed required posted documentation for Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. Smoke and carbon monoxide detectors were tested and are operable. Fire extinguisher indicated fully charged and is located in the dining area, and inspection tag expired 4/10/2021, deficiency cited. The home maintains telephone service via cell phone. The home is observed to be clean and orderly. There are toys and other age appropriate material available for children.

Children showing signs of illness are separated by different tables in the day care area waiting to be picked up by parental guardian.

Attached Garage was observed to have laundry machines, LPA observed chemicals stored in Garage, garage door is located in kitchen. Knives are stored in high inaccessible area. Per licensee, no children on medication at the day care and no poisons are stored in the home.

Outdoor area was observed children use side yard play area and per Licensee McDonald children are supervised at all times. No bodies of water was observed.

Facility has two infants in care, none present during LPA's visit. LPA observed play yards in day care area.

LPA reviewed (5) Children records and (3) staff records. LPA conducted interview with Licensee. Licensee has mandated reporter- expires 1/27/27. Licensee and Staff #3, pediatric first aid and cpr certification was reviewed and no EMSA sticker therefore not valid, deficiency cited. Staff #2 record was reviewed and did not have verification of Measles and Tuberculous immunization records in file, deficiency cited. Children records was reviewed and had appropriate documentation in file.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/resources/child-care-centers/. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Based on today’s visit, Per California Code of Regulation Title 22, (3) deficiencies were provided on LIC 809-D pages.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee Charisse McDonald.
