25 | On June 23, 2026, at 1:05 PM, Licensing Program Analyst (LPA) Keneisha Dunlap conducted an unannounced annual inspection of the facility. Licensee- La Creashia Willis granted entry after LPA Dunlap announced the purpose of the visit. At the time of inspection, 20 children were enrolled, with 12 children present. The operation hours are Monday- Sunday from 12:00a.m-12:00p.m. (Earliest time 4:00a.m.- 1l:00p.m. & 11:30-p.m.- 4:00a.m.) All adults in the home were discussed and background checked and fingerprinted.
The License, Earthquake Disaster Checklist (LIC9148), Disaster Plan (LIC610A), and PUB 394 (Parent's Rights) were all observed to be posted. The Licensee maintains a current LIC 9040 (facility roster). The most recent disaster drill was conducted on June 1, 2026, and documented in the disaster drill log.
This is a one-story home which consists of 3 bedrooms, 2 bathrooms, kitchen/dining room, living room, a detached garage, front yard, and fenced backyard.
Areas off limits include 3 bedrooms, 1 bathroom, kitchen, front yard, and detached garage.
LPA Dunlap observed the Living Room (Main Care Area): This area is equipped with a mounted TV, a sectional couch, a cubby locker for children's personal belongings, art supplies, and a ceiling fan. All furnishings, equipment, and materials were in good condition.
LPA Dunlap observed the second main care area is the dining area used for children in care. In the second main care room there is a Graco playpens, changing table, four highchairs, age-appropriate toys, additional art supplies, additional sleeping cots, and a wall mounted television. There is a mesh retractable baby gate in this area. All furnishings, equipment, and materials were in good condition. Page 1 of 5
