25 | Licensing Program Analysts (LPAs) T. Tran and A. Carter arrived at the above facility to conduct a Case Management Incident inspection to follow up on the self-reported incident that occurred at Apollo Center on 07/03/24. The Monterey Park SW Regional Office received the writing incident report on 07/08/24. Upon arrival, LPAs met with lead teacher, Candelaria Muniz. LPAs toured the facility and observed proper care and supervision. All center staff that was present during today’s inspection had fingerprint cleared and associated to the designated license number.
LPAs completed child record review. Staff files located at the main office not available at the site for review.
LPAs obtained child's document and personnel report. LPAs conducted interviews with staff, children, and other. Based on interviews conducted, it revealed that on the day of the accident there were two teachers supervising 7 children. During outdoor play, S2 observed C1 tripped and hit the forehead on the small bench. Child sustained a small bruise on the forehead and ice pack was applied. Parent contacted in a timely manner. Child received medical checkup due to a head injury and was released to school at full capacity on 7/6/24. Facility took extra measure by evaluated the play surfaces including child's footwear and no concerns observed. At this time based on the available information it does not appear this incident was the result of a Title 22 violation for lack of care and supervision.
The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited during today's visit.
A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with facility representative, Candeleria Muniz.
