Carter arrived at the above facility to conduct a Case Management Incident inspection to follow up on the self-reported incident that occurred at Apollo Center on 07/03/24. LPAs obtained child's document and personnel report. LPAs conducted interviews with staff, children, and other. Based on interviews conducted, it revealed that on the day of the accident there were two teachers supervising 7 children. During outdoor play, S2 observed C1 tripped and hit the forehead on the small bench. Child sustained a small bruise on the forehead and ice pack was applied. Parent contacted in a timely manner.
