Licensing Program Analyst (LPA) Warren Birks conducted a Case Management inspection. LPA met with Licensee Shohreh Maghoul who guided LPA on a tour of the facility. There were nine daycare children present (six preschool and three infants) being cared for by the Licensee and one staff.

The purpose of the visit is to amend a previous report to indicate that a function of the computer was unavailable to add an additional report. LPA created another report at a later date.

Note: LPA had computer malfunction during this visit.

Exit interview was conducted with Licensee Maghoul.. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as will result in a $100 civil penalty.
