On 03/15/2023 at 11:10 AM an unannounced Required – 1 Year Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. Facility is currently licensed for a capacity of 48. Licensing staff announced purpose of visit and initially met with Norma Garcia, Teacher, who granted entry to facility. Facility is located on Webster Elementary School campus in RM 28101 and RM 28102. The program is currently operating a Part Day Collaborative Co-Teaching (CCT) program in RM 28102 AM Program from 9:00 AM to 12:00 PM and PM Program from 1:00 PM TO 4:00 PM. Per Teacher and file review conducted, Full Day Program is currently not operating at this time which operates in RM 28101. LPA was taken on a guided tour of the facility of both indoors and outdoors. LPA observed required licensing postings on Parent Board inside RM 28102.

At the initial start of the site visit, there is a total census of four children and a total census of three teachers (S1, S2, and S3) present. Per Teacher, they currently have 22 children enrolled.

Beginning at 11:12 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed climbing structure and multi-level play structure with two slides that has cushioning material to absorb fall that are all well maintained. LPA observed a plastic play house, sensory tables, and covered cart for toys available for children. Required shade and fencing were inspected. LPA observed benches available for seating with shade available for children in outdoor play area. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to their drinking fountains during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Beginning at 11:18 AM, furniture and equipment were inspected for age appropriateness and good repair. LPA observed rugs and carpet through out to be free of hazards at time of inspection. LPA observed child size tables and chairs available for children well maintained. LPA observed learning stations for dramatic play, math, sensory area, and science with toys well maintained free of loose and sharp parts and a library

area available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Per Teacher, children do not nap at facility due to part time program offering. Availability of drinking water was reviewed. LPA observed personal water bottles available for children in classroom. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed two bathrooms available in classroom that each had one toilet and one sink, totaling two toilets and two sinks with a third sink available outside of the bathroom.

Menus were reviewed and observed posted at time of inspection. All meals and snacks are provided by the Long Beach Unified School District. Per Facility Representatives, AM Program is provided breakfast and PM Program is provided lunch which staff picks up from cafeteria. Trash cans used to discard food have tight fitting lids.

At 11:21 AM, LPA observed a first aid kit is kept in Teacher area and inventoried for necessary supplies. At 11:25 AM, carbon monoxide detectors were tested and observed operable at time of inspection. LPA observed smoke detectors present in the facility and fire extinguishers have been serviced on 07/13/2022. LPA observed disaster drill log available, last drill conducted on 01/2023. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. LPA observed these items stored under sink cabinet with operable lock requiring a key to open. Per Teacher, there are no children currently enrolled requiring medications. LPA observed isolation area for sick children labeled in Teacher area.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed all children present signed in electronically. Personal Rights of children were observed by LPA.

Beginning at 11:50 AM, staff and children’s records were reviewed. Children’s Records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857. The review of Staff records was documented on the LIC 859.

Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file. All staff have been given on the-job training sanitation principles, housekeeping, including pest management. LPA observed Pediatric First Aid/CPR expires on 07/10/2023 for S1. LPA did not review confidential staff files during the inspection due to the staff confidential files being stored at the administrative office. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 03/15/2023

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
