25 | Licensing Program Analysts (LPAs) Jeanette Estrada and Jonnisha Culbert made an unannounced Case Management Incident visit at the facility to follow up on two self reported incidents that occurred on 4/9/24 in regards to concerns with a violation of a child's personal rights and lack of supervision.
The Monterey Park South West Office received the Unusual Incident Reports via phone on 4/10/24. Upon arrival LPAs met with Staff 1. Present during the inspection were 15 napping children and 2 staff in the classroom. 2 additional staff arrived at a later time.
The first incident reported was in regards to a possible violation of a child's personal rights. Per the report, the parent of child 1 informed the facility that on 4/9/24 Child 1 stated that Staff 2 grabbed them, choked them and pushed them on the bed during nap time. Per interviews, Staff 2 was not present during nap time on 4/9/24. Staff 1 and Staff 3 were present during the nap time on 4/9/24. Per interviews conducted with Staff 1 and Staff 3, denied allegations. Per Staff 3, they did not witness Staff 1 do any of the mentioned actions.
The second incident reported was in regards to possible lack of supervision in the classroom. On 4/9/24 at approximately 5:00PM, Staff 2 had two children left for the day. They came into the classroom and Child 1 went to the computer station and child 2 went to use the restroom. At this time, Staff 2 was closing the blinds on the opposite side of the classroom from where child 1 was using the computer. Per Staff 2, they were then attending to child 2 when they looked up and didn't see child 1 at the computer any longer. Per Staff 2, they went to the restroom in the classroom to look for child 1 and didn't find them there. Staff 2 then went outside through the back door and saw child 1 running in the playground. Per Staff 2, the incident took no longer than 1.5 minutes.
During today's visit, LPAs conducted staff interviews and requested copies of pertinent documents. Further investigation will be conducted by CCLD. No deficiencies cited at this time.
Notice of site visit was provided and Site Supervisor was reminded it must remain posted for 30 days.
Exit interview conducted and a copy of the report was provided to Program Coordinator Lindsey Evans. |
