25 | Licensing Program Analysts (LPAs) Jeanette Estrada and Jonnisha Culbert made an unannounced Case Management Incident visit at the facility. The purpose of this inspection is to follow up on two unusual incidents that were reported to the Department on 4/10/24. There were 18 children and three staff present during today’s visit. LPAs met with Coordinating Teacher Randy Sanchez and informed him of the reason for the visit.
LPAs conducted a follow up visit regarding the incidents previously on 4/24/24. During the visit, LPAs conducted interviews with Staff and conducted observations of the facility.
Both of the incidents reported occurred on 4/9/24. The first incident reported involved a phone call made by the parent of Child 1 indicating that Staff 2 had grabbed them, choked them and thrown them on the bed during nap time. During the investigation it was revealed that Staff 2 was not present during nap time on 4/9/24. Staff 1 stated they were in the classroom on 4/9/24 during nap time. Staff 3 was also present. Per Staff 1, Child 1 was having trouble going to sleep and they put their arms around Child 1’s body, not around their neck. Per Staff 1, Child 1 was stating that they were being choked so they let go of Child 1 when they were near Child 1’s napping mat. Once Staff 1 let go of Child 1, Child 1 fell forward on their bed and stayed there during the rest of the nap time. Staff 3 who was also present during that incident stated that at no time did they see Staff 1 choke or throw Child 1 on the bed. Staff 3 corroborated the statements made by Staff 1. Since the occurrence of the incident, staff have attended a training on how to properly handle children.
The second incident reported was in regards to lack of supervision in the classroom with Staff 2. Per staff interviews, the facility has installed door alarms on both entrances of the classroom to alert whenever the doors have been opened and they have assigned two staff to be present in the afternoon at all times.
No deficiencies were issued during this visit. Notice of site visit was provided and Coordinating Teacher was reminded it must remain posted for 30 days. Exit interview conducted and a copy of the report was provided to Coordinating Teacher Randy Sanchez
