25 | Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced annual random inspection to the above facility. LPA met with Leslie Lopez, licensee who the purpose of this inspection was announced, and who guided analysts on a tour of the facility. Per Licensee, there are 6 children currently enrolled. A current children’s roster was available for review. There were 3 children present upon arrival- 2 being infants. Also present was 1 assistant and 1 adult daughter.
The licensee states that 5 adults and 1 child currently live in the home. Per Licensee, they currently have 1 assistant- which is her daughter. All adults present in the home have obtained a criminal record clearance or exemption prior to working, residing or volunteering in the licensed child care home. Licensee states that there are no firearms stored in the home.
This is a two -story home which consists of 5 bedrooms, 2 bathrooms, kitchen, dining room, living room, laundry room, front yard, back house and backyard (fenced). The children use the living room, bathroom in the hallway, and back yard.Per Licensee, areas off limits to children and parents include: upstairs, 3 bedrooms located downstairs, dining room/ kitchen (used as walkway only) laundry room and back house. LPA observed a baby gate at the bottom and at the top of the stairs making second floor inaccessible. The licensee provides food for children in care.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. There is telephone service via a cellphone that stays at the facility during operation hours. There is ventilation and heating (central). -------- pg. 1 of 4 ---------------- |
