On June 8, 2023, LPA Dayna Chambers conducted an annual continuation inspection which was not completed on June 6, 2023, due to technical difficulties. LPA observed a total of 9 preschoolers with 2 staff.
Licensing Program Analyst (LPA) Dayna Chambers conducted an unannounced annual inspection on June 6, 2023, at 12:45am. Director was not present until 1:15pm due to break. LPA met with Rudy Rodriguez, Director, who guided analyst on a tour of the facility. This is a preschool program which consists of 3 classrooms: RM #1-Leaping Lizards, RM#2- Bizzy Bees, and RM#3 Puddle Ducks. Facility operation hours are Monday to Friday from 6:45 AM to 6:30 PM. All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection: Room #1-Leaping Lizards: Staff #1 with 8 preschoolers. RM#2 – Bizzy Bees (Room is currently closed due to low enrollment), Room #3-Puddle Ducks: Staff #2 with 5 preschoolers. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility: Furniture and equipment were inspected for age appropriateness and in good repair. Telephone service, heating, lighting, and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are taken home each week to be washed. Napping equipment (mats) were observed in a separate storage room. Per director, the isolation area is in the office. Age-appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Director states that poisons are locked outside in the storage unit by custodian. Carbon monoxide detectors were observed and are operable. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal. Outdoor playground equipment is in a safe condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility. All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Children’s Records were reviewed. (The name, address, and telephone number of child’s authorized representative is on file) and medical Assessment.) for completeness; Inspection of required forms was made and documented on the LIC 857. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. LPA also issued the Review of Staff records (LIC 859) to the licensee during this inspection. The LIC 857 and the LIC 859 documents the staff and children’s files that were reviewed during this inspection. The center uses the Bright Wheel app for sign in/out and all the required information is present. Children's roster was reviewed and is current. Disaster drill log was available, last drill was conducted on 05/10/23 and 05/12/23. Parents provide meals and snacks to their children. First Aid supplies were observed in the classrooms. According to Director, medication is only administered to a child when accompanied with a doctor's note and is stored in the office. The facility was observed to be equipped with an isolation area for any child who becomes ill, and it is in the office. Per Director, the IMS statement needs to be updated. LPA provided topics to be covered.Medication: This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director submitted the completed lead testing reports and licensing forms on 07/22/22.There were no corrections needed. There were no deficiencies cited during inspection on June 6, 2023 or the annual continuation inspection on June 8th, 2023.
