On January 8, 2025, at 10:15a.m., Licensing Program Analyst (LPA) Keneisha Dunlap conducted an unannounced Complaint Inspection for the purpose of delivering findings for the above allegation. LPA Dunlap announced the purpose of the inspection and was allowed entry into facility by the Licensee Rosa Castro. During the course of an investigation conducted by LPA Dunlap, it was reported that the Licensee, restricts entry to authorized representatives of children in care. LPA Dunlap conducted interviews with the reporting parties, the Licensee, staff, and parents, and reviewed the children’s records.
Based on interviews conducted with reporting parties, access was in-fact denied to an authorized representative.
Five out of the five parent interviews conducted verified the Reporting Party account of being denied entry, but reported no significant issues with access to their children and stated that it is the policy of the facility that they do drop off and pick up only at door. They also stated that they are allowed to enter if needed.
During the staff interview, it was determined that the usual procedure was for parents to drop off and pick up children at the door. However, parents can come in if they request. Page 1 of 2

The Licensee stated that the entry policy -is that parents could enter "if they request to," the Licensee also emphasized that drop-off and pick-up typically occur at the door. The Licensee stated that parents "can enter" if they request to, but this is unclear and could be understood in different ways, which might cause confusion and make it difficult to follow the regulations.

Based on observations and interviews conducted, there is sufficient evidence to determine the alleged incident did occur. Given the available information, the allegation that the Licensee failed to allow an authorized representative into the facility is deemed SUBSTANTIATED.

A notice of site visit was given and must remain posted for 30 days.
