Licensing Program Analyst (LPA) R. Derraco conducted an unannounced annual inspection to the above facility on 03/14/25. LPA arrived at the facility at 09:15 AM and met with licensee, Elena Patterson, who guided analyst on a tour. Also present during this inspection was assistant S2. Per Licensee, there are 13 children that are currently enrolled. There were 9 children present upon arrival; Of the 9 children present, 6 children are infants. LPA advised licensee that a Type A citation under California Code of Regulation (CCR) section 102416.5(b)(1) and 102416.5(c) will be issued. S2 was observed to have background clearance however she is currently not associated to the facility roster. LPA advised licensee that a citation under Health and Safety Code (HSC) section 1596.871(c)(1)(A) will be issued. Individuals who reside in the home were noted and discussed. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

This is a one-story home which consists of 3 bedrooms, 3 bathrooms, kitchen/dining room, living room, a detached garage, front yard and backyard (fenced). Per licensee, the detached garage is part of an Additional Dwelling Unit (ADU) that has been certified for the City of Long Beach. Licensee was unable to provide any documentation regarding the ADU during the inspection. The off limit areas include the garage and the front yard

The main care are is located in the living room. LPA observed two couches, a diapering station a wall mounted television, and age appropriate toys. A fireplace was observed to be covered and locked with a fireplace cover. Per licensee, she provides food for children in care. The kitchen was observed to have locks (page 1 of 4)

beneath the kitchen sink. LPA observed a knife block and cleaning supplies on top of the kitchen counter. LPA advised licensee that a citation under CCR section 102417(g) will be issued. The dining area was observed with a dining table, dining chairs, and 4 Cosco high chair feeding tables. Bedroom 1 was observed with a bed, bedroom furniture and 2 pack n play playpens. Bedroom 2 was observed with a couch, computer desk, and two more pack n play play pens. The master bedroom was observed with a bed, bedroom furniture and two more pack n play playpens. All bathrooms in the home are designated for use by children in care. All bathrooms were observed to be clean with an operable sink, faucet and toilet. The outdoor play area is located in the backyard. LPA observed additional age appropriate toys, outdoor play equipment and patio furniture. Perimeter fencing was observed in the backyard and is used to separate the play area from the space designated for the ADU. LPAs did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs. Emergency Disaster Plan, Parent’s Rights Poster and the Facility License were observed to be posted.

All areas identified on the facility sketch that are accessible for children to use were inspected for safety,
comfort, and cleanliness. There is telephone service via a cellphone that is used and the cellphone stays at the facility during operation hours. Per Licensee, the home is equipped with central heating and air conditioning. The licensee understands that storage areas for poisons must be locked, not just inaccessible. LPA observed that the home is equipped with a 1A-10BC Fire extinguisher. LPA advised licensee that a citation under 102417(g)(1) will be issued. Smoke and carbon monoxide detectors were tested and are operable. No bodies of water were observed in the back yard play area. LPA observed 2 pet dogs and two pet cats on the premises. Licensee states that there are no firearms stored in the home. Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home.

The licensee has completed training on preventive health practices including Pediatric First Aid and CPR, however the CPR and First aid training certificate is not EMSA certified. LPA advised licensee that a citation under CCR section 102416(c) will be issued. LPA observed that the Licensee has proof of the Mandated Reporter AB 1207 compliant Child Care Training Certificate on file. File review was observed to have proper mandated immunization records. Children’s records were reviewed. LPA observed that C7 does not have a file. Per licensee, C7 has been attending day care for approximately 2 months. LPA advised licensee that citations under CCR section 102418(g), 102419(d), and 102421(b) will be issued. A current children’s roster was available for review.

LPA advised the licensee to access forms, regulations and quarterly updates online at: www.cdss.ca.gov.

The following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/resources/child-care-centers/.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
