Licensing Program Analysts (LPAs) Susann Sanchez and Angelica Wallin, conducted an unannounced annual required inspection at the above facility on at 1:30 PM. LPAs met with Vice Principal Authur, guided LPAs to the preschool where LPAs met Teacher Nare Karapetryan and guided LPAs on a tour of the facility. Preschool program operate 7:00 AM - 7:00 PM, Monday - Friday. The facility is adjacent to Scholars Preparatory School (private K-12 school). There is chain link fence between the preschool and school age facility. Each has its own entrance. The director's office is located near the entrance door to this facility. The ill/isolation area shall be in the director's office. A small bed is available for children. Founder Dr. Anna Grigoryan arrived at 2:45pm.

Founder states that there are no weapons or firearms on the premises. LPA did not observe swimming pools or spas on the premises. There are age appropriate toys and equipment on the premises.

At 1:45PM, LPAs toured the nap room. The following was observed and photographed:

  • 8 napping children and beds, making the blocking the walkways and 1 exit area.

At 2:00PM, During the file review LPAs observed and cited the following:
  • Staff Hasmik Gevorgyan is fingerprint cleared but not associated.

At 2:50PM, During file review. LPAs did not observed the following and Founder Dr. Grigoryan also stated they are missing the following:
  • Fire/Earthquake drills. LPAs reminded Founder that drills shall be conducted at least every six months.

Due to time constraints, LPAs will return at a later date to complete annual inspection and issue Type B citations that were observed in areas evaluated today.

Consult was given regarding TSP.

Based on the LPA’s observations, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22 Chapter 1 and Section CCR & H&S. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days. Appeal Rights were given. Exit interview conducted and report was reviewed with the Founder Grigoryan.
