Licensing Program Analysts LPAs Staicy Perry conducted an unannounced 3-Yr random inspection to the above facility. LPA met with licensee Anait Dagesian. A COVID risk assessment was conducted upon entry, no cases of covid present at the facility at the time of inspection. The licensee states that she currently has 24 children enrolled. There were 11 children present during inspection. A current children’s roster is available and is current. Licensee’s hours of operation are 7:00am-6:00pm, Monday-Friday.

The licensee provides care in the home. This is a two-story home which consists of 4 bedrooms, 1 and 1/2 bathrooms, kitchen, dining room, living room, backyard (fenced) and front yard. The children will use the entire downstairs area which consists of living room, dining room and half bathroom. The kitchen is the only area off-limits to children. Per licensee, areas off limits to children and parents include: The second floor, which consists of 1 bedroom, office, TV room, gym room and one bathroom. There is a door by the stairs which leads to the second floor to make the second floor inaccessible to the children. The licensee provides food for children in care. Parents/Guardians drop off the children through the front door and ring the Ring doorbell.



LPA toured all areas used by the children during this visit. All areas identified on the facility sketch were inspected. Family members residing in the home are 2 adults (Licensee and Licensees son) who have clearances on file and 0 children. LPA reviewed that all persons living in the home are identified on the attached LIC811. All individuals present in the home have obtained a criminal record clearance or exemption prior to working, residing, or volunteering in a licensed home. The licensee provides food for children in care. Licensee provides Breakfast, lunch, snack, and dinner. The licensee is on a food program called Evergreen. Licensee Anait also works with agency Pathways. Per licensee, there are no weapons, firearms or any bodies of water on the premises.

LPA observed age-appropriate toys and equipment present. Emergency Disaster Plan, Parent’s Rights Poster and the Facility License were observed to be posted. Sharp knives are made inaccessible by being placed in a high cabinet made inaccessible to the children.

At 1:26pm a fire extinguisher was observed. The valve on the required 2A 10BC fire extinguisher indicates fully charged but has not been serviced since 9/1/2022 as indicated on service tag. Per State Fire Marshall standards, fire extinguishers shall be serviced annually. LPA discussed the importance of ensuring the fire extinguisher is serviced or a new canister is purchased annually. Detergents, cleaning compounds medications, and other items which could pose a danger were made accessible to children. At 1:14pm LPA observed Lysol on a plastic drawer bin located in the dinning room area where the children play that is accessible to the children. LPA observed licensee remove the Lysol and place the Lysol away in a locked cabinet inaccessible to the children. LPA informed licensee that all medications, detergents, and cleaning compounds should be placed in a locked cabinet made inaccessible to children. Licensee understood. LPA observed the outdoor play area. Licensee uses the back yard for outdoor play. LPA observed a play apparatus with toys and equipment along with tarps to shade the children while outside playing.

The licensee states that there are no poisons in the home and understands that storage areas for poisons must be locked with a key or combination lock. At 1:14pm LPA observed Mosquito Repellant on a plastic drawer bin located in the dining room area where the children play that is accessible to the children. LPA observed assistant remove mosquito spray and placed it in the trash. There are first aid supplies available. Smoke and carbon monoxide detectors were tested and are operable. Licensee uses a cell phone for the home daycare that is left at the daycare at all times. Per licensee, chair in dquiet corner in living room will be used as an area for an ill child until an authorized representative comes to pick up child. LPA observed napping equipment in the form of cots for the children in care. The licensee is observed to be operating within the license capacity limitations.

The licensee has completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 12/8/25. Mandated reporter training expired on 10/14/2019. Proof of immunization against influenza, tuberculosis, pertussis, and measles were reviewed and on file. Licensee stated that she has an assistant Barbara who will assist when licensee should have an emergency. LPA perry inspected assistants file as well and file is complete. Assistant completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's assistants Pediatric First Aid and CPR expires on 12/8/25. Mandated reporter training expired on 10/14/2019. Proof of immunization against influenza, tuberculosis, pertussis, and measles were reviewed and is on file. Licensees’ son Manuk also was assisting. LPA reviewed file. Munak completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's son Munak has Pediatric First Aid and CPR expires on 12/8/25. Mandated reporter training expired on 10/14/2019. Proof of immunization against influenza, tuberculosis, pertussis, and measles were reviewed and on file. Children’s records were reviewed, including but not limited to, a copy of the emergency information card that contains all the information specified by regulation.

-There are no pets on the premises.
-Infant Walkers, Johnny Jumpers, Saucer Chairs, and/or any other item that falls into these categories are not permitted in a family childcare facility.
Smoking is prohibited in a license family childcare home.

Licensee Anait was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee Violeta. Licensee states that she has currently 2 infants enrolled. LPA discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.



Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee Violeta was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety
