13 | On July 23, 2025, at 9:30 AM Licensing Program Analyst (LPA) A. Carter conducted an unannounced Complaint Inspection for the purpose of delivering findings for the above allegations. LPA announced purpose of inspection and was allowed entry to facility by Maria Trujillo. There was 1 children present.
During the investigation LPA made observations, conducted interviews, and obtained records. Information gathered through interviews with staff and others uncovered conflicting information regarding the allegations. While the witness states the child had dried poop inside the child’s diaper at pick up, she did not bring concerns to the licensee’s attention and the licensee denies this incident occurred. Witness confirms, child was transferred to licensee’s care during transportation pick up with a bowel movement in his diaper, consenting to him not being changed until they reached the facility. LPA observed a diaper log at the facility, but the date of April 24th was not completed. Licensee states she was working alone and did not have time to log.
