25 | ANNUAL INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual inspection to the above facility on 07/11/2022. LPA arrived at the facility at 12:00 PM and met with Marine Mehrabyan, Licensee who guided analyst on a tour of the facility. Also present during this inspection, was Margarit Gaspparyan, Licensee’s Assistant. Per licensee, facility hours are Monday to Friday from 7:30 a.m. to 11:30 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee.
This is a one story home located on the first level. The home consists of 4 bedrooms, 2 bathrooms, kitchen, dining room, living room, garage, storage in the backyard, and backyard (fenced). The children use the bathroom in the hallway, living room, dining room, and front part of backyard (fenced) for outdoor activity. Per licensee, areas off limits to children and parents include: Kitchen, 4 bedrooms, 1 bathroom in the master bedroom, garage, and storage in the backyard. LPA did not observe any stairs in the home. Per licensee, parents enter the home through the main entrance door which leads to the living room. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 12:10 p.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room and dining room (daycare classrooms). LPA observed sofas, a coffee table, a TV, children's table, chairs, and toys in the living room and a dining table and chairs in the dining room. LPA observed a safety gate was installed to the kitchen to make the kitchen inaccessible to the children. Next LPA toured the bathroom in the hallway and observed child proof safety latches on the cabinets under the sink where licensee stores cleaning compounds. LPA observed off-limit bedroom doors where closed during this inspection. LPA observed the required 2A 10BC fire extinguisher was anchored to the wall in the hallway of bedrooms.
