On December 30, 2024, Licensing Program Analyst (LPA’s) Staicy Perry and Joanne Solorio Campos conducted an unannounced required annual inspection at Su-Ly Family Child Care. This is a large family child care home with a capacity of 14. Upon arrival, LPA’s disclosed the purpose of the inspection and met with Licensee, Ling Su-Ly who guided the LPA’s on a tour of the home. Adults present in the home all have criminal record clearance. There were 0 children present during today’s inspection(only licensee’s own 4 children were present). Licensee states that there are currently 8 children enrolled. The children's roster was reviewed and is current. Per licensee, the facility’s hours of operation are 7:30am to 5pm. Emergency Disaster Plan, License, and Parents’ Rights were posted at the time of inspection.

This is a one-story home apartment complex, unit A, which consists of 3 bedrooms 2 bathrooms, kitchen, and living room. Areas used by the children include the living room and 1 restroom. Per Licensee, areas off- limits to children include all bedrooms, 1 bathrooms, and the kitchen. LPA observed that there is a baby gate separating areas used by children and off-limits area. Per licensee, off limit areas are also locked during operating hours, LPA observed doors to be locked. LPA’s observed a fireplace located in the living room and is made inaccessible to prevent access by children.

All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety of the children. There is a working telephone maintained in the home. LPA’s observed age-appropriate toys, free of loose and sharp parts. LPA’s observed electrical cover outlets all through the home. Detergents, cleaning compounds, medications, and other items which can pose a danger to children are inaccessible to children and locked under the sink. The Licensee states that there are no poisons in the home. The Licensee does understand that poison must be locked, and LPA’s Perry and Campos did not observe any poisons during this visit.

Per licensee, food is provided by licensee and used a food program called Options. Licensee was advised that if food is brought from the children’s homes, all containers must be labeled with child’s name and properly stored or refrigerated. Licensee provides am snack, lunch and PM snack. Licensee does not transport any children.

LPA’s did not observe any pets at time of inspection and Licensee confirms she has no pets. Per Licensee, there are no weapons, firearms in the home and there are no bodies of water around the premises. LPA’s did not observe the above mentioned at time of inspection.

The valve on the required 2A 10BC fire extinguisher indicates fully charged and was observed in the kitchen area of the home and last serviced on 9/11/24. LPA observed an operational smoke and carbon monoxide detector located in the hallway and are in operable condition. LPA observed First Aid kit and was inventoried for necessary supplies. Disaster drill was last conducted on 10/17/24.

Licensees’ and licensee’s assistant’s records were reviewed. The licensee and assistant have completed training on preventive health practices dated 5/21/17 including Pediatric First Aid and CPR. Pediatric First Aid and CPR expires on 5/30/25.

LPA’s did observe that Licensee and assistant to have proof of Mandated Reporter AB 1207 compliant Child Care Training Certificate on file. LPA’s reviewed children’s records, including emergency information, and were observed to be complete.


The following were discussed:
Rooms that are off-limits need to be made inaccessible during operating hours. NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers and any other item that falls into that category are permitted in the facility. Effective January 1, 2010, licensees of family child care homes are required to ensure that at least one staff member with current training in pediatric first aid and pediatric CPR is on site at all times when children are present.

Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. LPA reviewed and issued the LIC 311D - Forms/Records to Keep in Your Family Child Care Home. Mandatory Forms for the children’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Licensee was advised how to access forms and Regulations online at www.ccld.ca.gov. Licensee was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care.

Licensee Ling Su-Ly was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee Ling Su-Ly and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee Ling Su-Ly of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA’s observed the safe sleep log and LIC 9227 to be on file and complete as licensee does care for infants 0-12 months and were found to be complete.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Family Child Care Homes Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.



Centers and Family Child Care Homes Licensee Ling Su-Ly was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the licensee Ling Su-Ly confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited on today’s visit.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with the Licensee Ling Su-Ly.

Appeal rights were provided and discussed.

Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days.
