Per interviews conducted and review of documents it was stated and observed that yes the preschool children are combined with the toddler children if a staff needs to use the bathroom or breaks and also in the morning when both the preschool and toddler children arrive they are sometimes commingled.
Based on interviews, disclosures made during the department investigation and documents reviewed and received, the preponderance of evidence and the standards have been met, therefore, the above allegation is found to be substantiated. California Code of Regulations (Title 22 Division & Chapter) are being cited on the attached deficiencies page LIC 9099-D.
Consultation was given on this date regarding commingling of children, staff days and hours, and when a therapist conducts a visit and use of an empty preschool classroom.
Exit interview conducted and report was reviewed and given to Tania Torres, Director.
A notice of site visit was given and must remain posted for 30 days.
LPA, Cynthia Reyes informed, Tania Torres Director that this report dated 08/13/2026 document(s) (1) Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety or personal rights of a child in care. Also, LPA, Reyes informed, Tania Torres Director to provide a copy of this licensing report dated 08/13/2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months (1 year) from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9224), or written statement, must be placed in the child’s file for verification. The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. LPA provided Tania Torres, Director with a copy of the LIC 9224. |
