25 | On 3/28/24 at 9:00 AM Licensing Program Analysts (LPA) Claudia Kam and Katrina Chicote met with assistant director Jacquelyn Galvez-Chacon and conducted an Unannounced Case management Inspection to follow up on multiple incidents that was reported to the Department on 3/14/24 and 3/19/24. Upon arrival, LPAs announced purpose of inspection and met with Facility Representative, who granted entry to facility. Census was taken and 15 children were present at time of visit.
On 3/14/24, an incident was reported to The Department where a child sustained an injury while in care. on 3/19/24, an incident was reported regarding a parent who was disruptive in front of day-care children and staff. The facility reported these incidents to the Department within the required 24 hours and submitted written reports within seven days.
During this inspection LPAs interviewed staff, observed room incident took place in, and observed child named in report. Director has detailed that the child did hit his head and the ambulance was called due to medical issues reported by the parent on 3/14/24. File was reviewed and medical treatment documentation in file. No IMS is provided to child as any medicine is provided by parent or doctor. Assistant Director has been directed by parent to call 911 if child has any medical episodes as diagnosis is pending. Assistant Director and Teacher stated this incident occurred after drop off and that 911 and parent were called immediately. Child returned to care the next day. LPAs observed child present at the facility and active.
LPAs interviewed Assistant Director who was the primary regarding incident on 3/18/24. Director has detailed that there were 2 incidents on 3/18/24. Parent displayed aggressive behavior such as screaming and making threats at staff during drop off and pick up due to issues with dropping off a stroller. Assistant Director states she informed Parent that she could not leave the stroller in the hallway of the facility due to it being a safety hazard. Per assistant director the parent was allowed to leave the stroller on the Friday as a one time courtesy and when denied the ability to drop off stroller she continued to display aggressive behavior and called the main office to complain.
