On February 19, 2025, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management- Deficiencies inspection at the above facility. LPA met with director, Michele Masjedi who guided LPA on a tour of the facility. LPA observed 8 infants in care with 4 staff.
The purpose of this inspection is to issue a Type A deficiency in accordance with Title 22 Regulations, Personal Rights 101223(a)(3). During the course of an investigation, LPA reviewed video footage with LAPD on 01/21/2025 and on 02/10/2025. While reviewing video footage it was observed that on 11/19/2024 at 10:43 AM, Staff #1 (S1) walked into the napping room while an infant was laying down. The video footage shows S1 press on the infant’s side/pelvis area, squeezed the infant’s cheeks and abruptly lifted the infant out of the crib. Though the video footage did not include audio recording, the infant clearly reacted by showing motions of distress (crying).
LPA Ayala informed the director, Michele Masjedi to provide a copy of this licensing report dated 02/19/2025 that documents any type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report, A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child’s file for verification.
An exit interview was conducted and a copy of this report was provided along with Appeal Rights. A Notice of Site Visit was provided and must be posted for 30 days. |
