On 03/22/24 Licensing Program Analyst (LPAs) Staicy Perry and Claudia Guangorena conducted an unannounced 3yr. Annual Inspection to the above facility. Upon arrival at 12:15pm, Licensing staff met with Coach Coordinator, Golnaz Kooklanfar who greeted licensing staff in the lobby and took staff to first floor where facility is located. Lead Teacher Vivian Castaneda greeted licensing staff. Shortly after Ana Marquardt Associate Director of Education arrived. This is an infant program with a toddler component licensed to serve 18 children, 12 infants and 6 toddlers. Waiver is on file for this facility for shared outdoor activity space. Facility has 3 Pod classrooms. Per Lead Teacher, there are currently 2 toddlers enrolled in the program. Hours of operation for this facility are Monday through Friday from 7:30 AM until 3:00 PM.

Licensing staff was led on a tour of the facility. Licensing staff observed all required forms/publications to be posted by the front entrance. Waiver was not observed to be posted. LPA Perry reminded staff that waiver should remain posted near facility license at all times. Food menus were reviewed to ensure that they are being posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Facility provides breakfast, lunch and PM snacks. Facility uses food program with CACFP.

PHYSICAL PLANT: All areas identified on the facility sketch were inspected. Upon arrival, licensing staff observed all Pods. (classrooms) Pod 1 had 0 children and 0 staff. POD 2 had 2 napping children and 1 staff and Pod 3 had 0 children and 0 staff present. Teacher-child ratios were observed, and staff names were recorded. All children were observed to be under visual supervision of a teacher at all times. The Licensee is within the conditions, limitations, and capacity specified on the license. Individual storage space was observed available for each child inside the classrooms. Toys were observed to be clean and safe. Furniture and equipment were inspected for age appropriateness and good repair. There are smoke detectors and fire extinguishers located throughout the facility.

Facility is also equipped with cameras that facility has live and recorded access to at all times. Licensing staff observed the facility to have 2 toilets and 2 sinks available to children. All toilets and sinks are in safe and sanitary operating conditions. Licensing staff observed Pod 1 and Pod 2 to have a changing table within arm’s reach of a sink. Licensing staff did not observe Pod 3 to have a sink within arm’s reach of the diaper changing table. Pod 3 sink was positioned in an area behind the diaper changing table making it inaccessible to reach. During inspection licensing staff observed facility maintenance come in and rearrange diaper changing area to be near the sink. Licensing staff did observe the Diaper changing area be within arm’s reach of a sink. The facility was observed to have diapers and wipes available in each classroom near the changing table. Teacher-child ratios were observed to be in accordance with Title 22 regulations. The Licensee is within the conditions, limitations, and capacity specified on the license. LPAs reminded Lead Teacher that all children need to be observed to be under visual supervision of a teacher at all times. Napping equipment was in the form of cots and bedding were inspected for good condition, appropriate storage, and cleanliness. Children's napping sheets and blanket are kept in child’s cubbies as well and is washed on a weekly basis by the facility and as needed. Storage for children's belongings were inspected. All toilets and washing facilities are in safe and sanitary operating conditions. Facility was observed to have one staff bathroom with cleaning solutions that is placed high up.

All classrooms were observed to and inspected for health and safety. First Aid supplies are available and complete in each classroom. Drinking water was readily available indoors in a blue Rubbermaid jug with small disposable cups that are accessible to the children.

At this time, the main office is used as an isolation area, there is a cot available for an ill child to rest and a staff restroom nearby.

Licensing staff observed disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Medication will be placed in a locked container with a latch inside each Pods Emergency Bags. Classroom furniture and equipment are in good condition, free of sharp, loose, or pointed parts. All floors are clean and safe. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. The facility was observed to be free of flies, other insects, and rodents.

Licensing staff observed the valve on the required 2A10BC fire extinguisher indicates fully charged and was serviced on August 22,2023, as indicated on service tag. Per State Fire Marshall standards, fire extinguishers shall be serviced annually. Facility has functioning carbon monoxide detector in the classroom and smoke detector that meets statutory requirements. Lead Teacher states there are no weapons, firearms, or bodies of water on the premises.

OUTDOORS: Licensing staff reviewed the outdoor play area. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Per Administrator play structure is off limits due to pending city permit due to the addition of shade that was added over the play structure. Licensing staff observed bikes, lounge furniture that is age appropriate and playhouses for children in care. Drinking water is readily available outdoors in a gallon Igloo container with small cups available for the children while outside play.

FACILITY RECORDS: Sign in and out sheets were reviewed and are placed inside each classroom at the entrance of for parents to sign in and out daily to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Disaster drills have been conducted at least every six months. Last drill conducted was March 21, 2024.

There is at least one person trained in Pediatric CPR and Pediatric First Aid present during this inspection. Licensing staff also reviewed staff records. The review of Staff records was documented on the LIC 859. Staff present had proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present had proof against TB, measles, pertussis, and influenza.


Children’s Records were reviewed for completeness; Inspection of required forms was made and documented on the LIC 857.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Licensing staff discussed the safe sleep regulations with Lead Teacher and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Lead Teacher of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).


Licensing staff referred director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. Facility has tested for lead and had no exceedances. LEAD results are posted.

Lead Teacher Vivian was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
