25 | Licensing Program Analyst (LPA) Stephanie Li conducted an unannounced Case Management inspection due to an incident that occurred on 4/9/24 and was reported to the Department on 4/10/24. The preschool head start program was closed for the summer. 0 Preschool children were present, but the toddler and infant program was operating. Toddler and infant teachers confirmed that the previous Preschool lead Teacher was no longer working at their site since the end of last school year.
On 4/12/24, an unusual incident report was made to the department regarding Head Start Lead Teacher and Para professional teacher. LPA contacted Head Start Supervisor Laura Borja. Laura confirmed that Lead Teacher was placed on administrative leave on April 15, 2024 and did not return to employment. Laura confirmed that the Para professional Teacher was also placed on administrative leave a week after. Supervisor stated that it is not decided if para professional will be returning in the fall. Based on information obtained during this investigation, no additional follow up is necessary regarding the incident reported.
No deficiencies are being cited at this time.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
