25 | Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced required 1- year inspection to the above facility on 09/08/23. LPA arrived at the facility at 12:20 PM and met with Andrea Mary Karis, Licensee who guided analyst on a tour of the facility. A COVID 19 risk assessment was conducted prior to entering the facility. Also present during this inspection, were Rebecca Gourley and Sandi Vega, Licensee’s Assistants. Per licensee, Andrea Karis facility hours are Monday to Friday from 8:30 a.m. to 5:30 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee.
This is a one story home located on the first level. The home consists of 2 bedrooms, 2 bathrooms, kitchen, dining room, living room (FIREPLACE: inaccessible), laundry room, front yard, side yard and garage (storage). Per licensee at 12:30 PM, the children use the living room, dining room, one bedroom, one bathroom in the hallway, and front yard (fenced). Per licensee areas off limits to children and parents include: Master bedroom, bathroom in the master bedroom, kitchen, laundry room, and garage (storage). LPA did not observe any stairs in the home. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report. Per licensee, parents enter the facility through the main entrance door which leads to the living room.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 12:30 p.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room and dining room (daycare classrooms), LPA observed children chairs, tables, three high chairs and toys in the living room and dining room. LPA observed 7 children, two being infants were having lunch in the dining room with license and her two assistants. LPA observed fireplace in the living room was blocked off to prevent access. LPA observed all electrical outlets were covered in living room and dining room. Next LPA toured the daycare bedroom and observed two cribs. Next LPA toured the daycare bathroom in the hallway, it was clean and sanitary. LPA did not observe any hazards in the bathroom. LPA observed a child safety gate was installed in the hallway of bedrooms to make the off-limit master bedroom and off-limit bathroom in the master bedroom inaccessible to the children.
