25 | Licensing Program Analysts (LPA) Anomeh Eivazian and Stephanie Li conducted an unannounced Plan of Correction inspection to the above facility on 10/19/23 to ensure Type B deficiencies issued on 09/08/2023 were cleared. LPAs arrived at the facility at 2:50 PM and met with Katrina Paramazyan, assistant who guided analysts on a tour of the facility. During this inspection Licensee, Andrea Karis was not present in the home. Also, Laura Duran and Rececca Gourely, assistants were present in the home.
During this inspection at 3:16 PM, LPA Eivazian talked to Brenna Karis, licensee's daughter over the phone and informed her based on Title 22 Regulations code: 102369(b)(9)-- Application for Initial License--Evidence of a current tuberculosis clearance, not more than one year prior to or seven days after initial presence in the home, for any adult in the home during the time that children are under care, TB Test clearance is required for all adults present in the home during daycare operation hours.
Licensee did not have current Pediatric First Aid and CPR certificate and TB Test Clearance for all staff on file during this inspection which were cited on 09/08/23. Also, Brenna Karis stated that licensee, Andrea Karis was not able to find a vendor to retake her required pediatric first aid and cpr training.
During this inspection 9 children were present in the home, three being infants, napping in the living room.
The following were observed during this inspection:
- Staff ratio and capacity has been met. During this inspection, there were 9 children in care with 3 staff.
- Licensee has current required Mandated Reporter training certificate on file, completion date: 09/13/23.
LPA informed Katrina Paramazyan, assistant that licensee's current Pediatric First Aid and CPR certificate and all staff TB test clearance are required to be submitted to LPA Eivazian by 11/10/23.
The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today from the areas that were inspected today.
