Licensing Program Analyst (LPA) Susann Sanchez conducted a case management inspection to amend the 9099 report issued on 08/15/24. LPA was greeted by administrator Maria Aguado and LPA was provided guided tour of facility. Census was taken.

LPA Sanchez reviewed amended 9099 report with licensee and provided a copy of the amended report. LPA collected wet signature on the 9099 dated 08/15/24.

The Notice of Site Visit (LIC 9213 ) was provided to the Licensee. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00 Exit interview was conducted with Licensee Maria Aguado and a copy of this report was provided.
