25 | ANNUAL INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual inspection to the above facility on 01/13/2022. LPA arrived at the facility at 8:40 AM and met with Hasmik Torchyan, Licensee and Narine Hovhannisyan, licensee's daughter inlaw/assistant who guided analyst on a tour of the facility. Per licensee, Hasmik Torchyan facility hours are Monday to Friday from 7:30 a.m. to 5:30 p.m.
This is a one story home which consists of 3 bedrooms, 2 bathrooms, kitchen, dining room, living room, garage (limited activity space), detached guest house which consist of one room, kitchen and 1 bathroom, front yard, and backyard (fenced). The children use 1 last bedroom (day care room), 1 bathroom in the day care room, and backyard (fenced), and detached garage for limited activity only. Per licensee children do not eat or sleep in the detached converted garage. Parents enter the facility through the side gate which leads to the day-care room and backyard. Per licensee, areas off limits to inaccessible to the children. LPA did not observe any stairs in the home. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 8:50 a.m., Licensee's daughter in law, Narine Hovhannisyan took this LPA on a tour of the home. LPA was toured through the interior starting with the daycare bedroom where LPA observed children toys and napping cots and one pack'n pkay in the bedroom. Next LPA toured the bathroom in the bedroom and did not observe any hazards. LPA observed a safety gate was installed to the kitchen to make the off-limit areas in the home inaccessible to the children. Next LPA toured the detached converted garage (for limited activity only) and observed children toys, chairs and tables in the detached converted garage.
