On March 14, 2023, at 7:25 a.m., Licensing Program Analysts (LPA) Denise Gibbs and Susann Sanchez conducted an unannounced inspection and met with Licensee, Angela Huffman.

There were eight children and two staff present when the visit began.

During investigation LPA's toured the facility and reviewed records for 11 children. Information provided in the files and on the Children's Roster show that two children are under 18 months old. Staff confirmed that care is being provided to two school age children, who have no files and are not on the roster. Staff walk the children to the nearby elementary school a minimum of two days a week. According to Title 22, facility is working outside of its licensed capacity, which is an immediate risk to children in care.

While touring facility LPAs observed floor boards that are not flush to the floor in the main entrance walkway. This is a potential tripping hazard for children in care. ------------------Page 1

Based on the available information, the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be Substantiated. California Code of Regulations, 101238(a) Buildings and Grounds, are being cited on the attached LIC. 9099D.

Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit.


The Notice of Site Visit (LIC 9213) – must remain posted for 30 days.

Exit interview was conducted with Director, Christian Seymour. A copy of this report and appeal rights were discussed and left with Director.

On March 14, 2023, at 7:25 a.m., Licensing Program Analysts (LPAs) Denise Gibbs and Susann conducted an unannounced complaint inspection and met with Licensee, Angela Huffman.

There were eight children and two staff present when the visit began.

During the investigation LPAs obtained a copy of the children's roster, Personnel Report, Sign in/out sheets, took pictures and conducted interviews with four staff.

LPAs observed both toilets in the children's restroom were operable on today's date. Per Staff, one toilet was clogged one day last week, date unknown, and toilet was fixed the same day. License capacity is for 15 children and according to staff one toilet was still available for children.

While LPAs toured the facility there was no foul odor in the classroom. Per Staff, when children have bowl movements, they wrap the diaper in a glove and dispose of the diaper in the outside trash can. -------Page 1

LPAs observed the entrance gate on 107th st. Gate was observed to operable. LPAs did not observe a broken hinge on the gate.

Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove
the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.

Per Direcrtor they will communicate with parents that teh school age children and infants in care can no longer come to the facility. Licensee will type a letter letting the parents know the situation and email the letter to LPA by POC date 3/15/23

Based on observation the licensee did not ensure the facility operated within its limitations. Licensee is providing care for Infants and School-Age children. This poses an immediate health and safety risk to children in care.
