32 | Based on LPA Calderon's record review of the facility Parent Handbook it did not include information on diaper procedures or related service that indicates facility requires to do diapering services at the facility. Per Reporting Party (RP) email that LPA Calderon received it did not indicate time frame indicating dirty diapers observed on children in question C7, C8 and C9; furthermore it was reported by RP children C7-C9 were picked up at the day care by individuals from HSA who would smell urine on child Child #7 (C7). Based on email review there was no indication HSA individuals physically observed dirty diapers or evidence to state dirty diapers occurred at the day care or outside of day care premises . During interview Director, Staff #1 (S1), Staff #2 (S2) and Staff #3 (S3) during interviews informed LPA they change children's diapers every 2 hours . Director Christian Seymour informed LPA Calderon that the children in question C7, C8 and C9, would be a the facility from 8-9:30am, once a week, they would get picked up to go on parent visits and they made sure their diapers were changes before leaving the facility. The children in question based on Director's statement children were not present for the full two hour period but received diaper changes. LPA reviewed diaper logs, diaper log for children in question were not logged every time children were present and per Parent Handbook review it does not indicate diaper logs are a facility requirement. Foster mom of children in question informed LPA Calderon C7-C9 received diaper changes and did not observe issues with dirty diapers. There was no evidence to demonstrate that children in question diapering needs weren't met, therefore this allegation is unsubstantiated.
Based on allegation: Staff do not ensure child's hygiene needs are met while in care. Based on LPA Calderon record review of Parent Handbook states " keep in mind, your children will participate in activities that may cause them to dirty their clothing, such as painting, arts and crafts and outside activities so playing clothes will be best for day to day activities." Per email from reporting party (RP) reply to LPA Calderon's email, when asked if they saw children in question neglected and dirty themselves, RP stated " the children have been observed dirty outside of the daycare by myself and from their supervisor and the HSA individuals and therapist have reported to observe the children dirty directly from the daycare. LPA during visit to the facility did not observe children C7-C9 at the facility. LPA attempted on 5/7/26 to interview children but was not successful. Day care children present at the facility on LPA Calderon's visit date 3/11/26 were observed staff instructing children to wash hands and face wiped during before and after meals were given to the children, in addition LPA observed hygiene practices being implemented and observed, lunch time, potty breaks and hand washing.
