On 03/05/25, Licensing Program Analyst (LPA) Tyler Reyes and Ashley Calderon conducted an inspection at the facility to review the Plan of Correction (POC) and ensure compliance with Title 22 regulation on the citations that were cited on 01/06/25 and 02/20/25. LPAs disclosed the purpose of the inspection and was granted entry in the facility by Administrator/ Teacher Christian Seymour.

Facility operation hours are 5am-5pm Monday – Friday and 5am-5pm Saturday-Sunday (if needed). This facility consists of 1 classroom and outdoor play area. This facility is licensed to have Preschool Program serving ages 2 until entry into 1st . LPAs observed (12) twelve children and (2) two teachers and (2) two assistants.

LPAs reviewed the Carbon Monoxide detector and confirmed it is operational. Additionally, LPAs verified that the facility conducted its most recent emergency disaster drill on 02/26/25. During the inspection, LPAs observed children napping. LPAs observed that sheets and blankets were stored in individual bags and kept separate, ensuring proper storage. LPAs observed the facility’s posted food menu posted on wall near the entrance.

LPAs received two (2) signed and dated declarations from Christian. The declarations specify which staff member is responsible for maintaining the sign-in and sign-out sheet. Additionally, the facility confirmed that the toddler component has been removed.

No deficiencies were cited during today’s visit. An exit interview was conducted, and a copy of this report and appeal rights was provided to the facility. Notice of Site Visit must remain posted for 30 days. Failure to do so will result in a $100.00 civil penalty.
