Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced Case Management inspection on this date as requirement of three (3) years probation beginning on 10/07/21. LPA arrived at 9:37 at the facility and met with Sandra Benitez, Licensee, who provided a tour of the facility. Upon arrival LPA observed 7 children supervised by licensee and 2 assistants. A representative from MAOF was present during inspection. Also present was licensee's adult son. Individuals residing in the home were discussed and noted. All adult working with children and residing in the home have obtained a criminal background clearance.

All areas on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness.

Licensee and staff files were reviewed, Licensee maintains a current facility roster. Licensee CPR/First Aid expires 09/2023. Children and staff files were reviewed and found complete. The required posting including the facility license and Parent's Rights poster were observed in a prominent location in the living room.

Toxins and sharp items are stored in kitchen and locked bathroom cabinet inaccessible to children. Licensee was advised to frequently check the safety locks and gates as they can break with wear and tear.

Licensee maintains posted in a visible area in the dining room.

LPA inspected the enclosed outdoor play area in the backyard. Adjacent to the play area is side yard, area that includes a storage unit, and a tall metal ladder. LPA advised licensee to ensure the gate leading to the side yard is always closed and locked.

LPA informed licensee on the following: keeping in compliance with Title 22 Regulations, criminal record clearances, mandated reporting, reporting requirements, supervision, terms of license, and record keeping.

Based on record reviews and LPA observations, no deficiencies were cited during this inspection.

Exit interview conducted with licensee, Sandra Benitez.
