An unannounced Required – 1 Year Inspection was conducted on 12/08/2022 at 11:35 AM by Licensing Program Analyst (LPA) Katrina Chicote. Facility is currently licensed for a capacity of 225 but currently have 106 enrolled. Licensing staff met with Maria Harris and Sonia Gutierrez, Facility Administrators. The program currently operates Monday thru Friday from 7:45 AM to 2:45 PM. LPA was taken on a guided tour of the facility of both indoors and outdoors. Required postings were observed posted in school lobby and in each classroom.

Facility is composed of ten classrooms but facility is currently using nine classrooms at this time. LPA observed in RM 4110: 13 preschoolers with three staff(S) S1, S2, and S3, RM 4108: 11 preschoolers with two staff (S4 and S5), RM 4107: two preschoolers going down for nap with one staff (S6), RM 4104: seven napping preschoolers with two staff (S7 and S8), RM 3103: seven napping preschoolers with two staff (S9 and S10), RM 2110: five napping preschoolers with three staff (S11, S12, and S13), RM 2107: 16 napping preschoolers with three staff (S14, S15, and S16), RM 2106: 16 napping preschoolers with two staff (S17 and S18), RM 2104: nine napping preschoolers with two staff (S19 and S20). At the initial start of the site visit, there is a total census of 86 children and a total census of 20 teachers present.

Beginning at 11:45 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. LPA observed learning stations in each classroom with toys filled for dramatic play, manipulatives, art, and other age appropriate toys free of loose and sharp parts. LPA observed quiet play area available for children in each classroom with soft seating and books available. Floors and rugs in classrooms observed to be free of hazards and well maintained at time of inspection. Mats/cots were inspected. Per Administrators, linens are provided by the facility and washed weekly, parents provide additional blankets that are taken home weekly. LPA arrived during nap time and observed cots being used in each classroom at time of inspection with cots placed that does not block walkways or access to exits at time of inspection. classroom and children are provided cups. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed the following classrooms to have shared bathrooms each with three toilets and two sinks: RM 4110 and RM 4109, RM 4107 and RM 4104, RM 2110 and RM 2107, and RM 2106 and RM 2104. LPA observed two additional sinks (four total) inside each classroom.

A first aid kit is kept in a backpack hung by the door that is taken inside and outside. LPA observed carbon monoxide detectors and smoke detectors are present in each classroom. Fire extinguishers have been serviced on 11/2022. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. LPA observed these items stored in high cabinet in teacher area. LPA observed medications for children in RMs: 4110, 3103, 2110, 2106, and 2104 all stored according to Title 22 Regulation requirements in high cabinet in teacher area. Disaster drill log was available. The last drill was conducted 11/17/2022.

Snack/lunch menus were reviewed and observed posted in each classroom. Per Administrators, food is delivered from nearby Barton Elementary School cafeteria and prepped on facility's kitchen for distribution. At 12:29 PM, LPA toured food preparation area for general safety, sanitation and proper equipment. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Facility serves Breakfast, Lunch, and Snack.

Beginning at 12:14 PM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed climbing equipment with slide and steps with cushioning material below to absorb fall all well maintained. LPA observed various outside play areas for music, building blocks, and dramatic play area with plastic play houses all well maintained. Required shade and fencing were inspected. LPA observed benches with umbrella for shade at time of inspection. LPA advised Administrators that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to drinking fountains during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. All children present were signed in with full legal signatures. Personal Rights of children were observed by LPA. Report Continues - Page 2 of 3

All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District.

Beginning at 1:30 PM LPA conducted file reviews for children and staff. Children's roster was reviewed and is current. Children's records were reviewed for completeness. Staff present have proof of the AB 1207 Mandated Reporter Training certificate on file. CPR card expires on 01/2023 for S19. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. LPA did not review confidential forms and information due to the files being stored at the administrative office. Per Administrator, sick/isolation area is in a room next to Administrator's office.

Incidental Medical Services:
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 12/08/2022.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
