Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced required 1 year inspection on 08/27/21 at 8:50AM. LPA met with Sonia Gutierrez, Program Specialist & Kelly Sowle, Mentoring Supervisor, who guided analyst on a tour of the facility. This is a preschool program which consists of 10 classrooms; Rooms 4107, 4104, 3103, 2110, 4110, 4108, 3101, 2107, 2106, and 2104. Facility operation hours are Monday through Friday from 7:45 AM to 2:45 PM.

All areas identified on this report were inspected. Upon arrival, the following staff were present during this inspection: Room 4107: Staff #1, #2 and #3 with 5 preschoolers; Room 4101: Staff #4 and #5 with 5 preschoolers, Room 3103: Staff #6 and #7 with 3 preschoolers; Room 2110: Staff #8 and #9 with 4 preschoolers; Room: 4110: Staff #10 and #11 with 14 preschoolers; Room 4108: Staff #12, #13 and #14 with 12 preschoolers; Room 3101: Staff #15 and #16 with 13 preschoolers; Room 2107: Staff #17 and #18 with 8 preschoolers; Room 2104: Staff #19 and #20 with 12 preschoolers, and Room: 2104 had no children or staff present. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linen is washed at the facility weekly. Napping equipment (cots) were observed in a separate storage rooms. Per Program Specialist, the isolation area is located in the nurse's office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. During the inspection LPA observed that water faucets are available in each classroom and accessible for children to drink from. LPA advised that the drinking faucets be covered and children be given water from disposable cups due to the current pandemic in an effort to reduce the risk of contracting the virus. Availability of indoor drinking water was observed in classrooms. -------Page 1 of 4

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Per Program Specialist, there are no poisons stored at the facility. Carbon monoxide detectors were observed and are operable.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

All individuals present have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed. In review of children's files, LPA observed that forms in which a signature is required were missing signatures. Per Program Specialist, the facility decided to accept enrollment packets electronically due to COVID-19. While inspecting the forms LPA observed that there was no electronic signatures or physical signatures on the forms. The forms indicate "over the phone." When LPA inquired as to why the forms indicate "Over the phone" Program Specialist informed LPA that consultation was conducted over the phone and parents were emailed the forms to keep and read. LPA informed Program Specialist that forms such as the LIC 613A: Personal Rights require a signature from the parent or guardian and since the facility failed to obtain a signature and inform the department of their intent to obtain electronic enrollment packets that include licensing forms, this poses a potential health and safety risk to children in care.

LPA also reviewed staff records. The review of staff records was documented on the LIC 859. Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file with the exception of Staff #16 whose certificate was expired. Staff present did have proof against TB, measles, pertussis, and influenza. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. LPA issued the Review of Staff records (LIC 859) to the Program Specialist during this inspection. The LIC 857 and the LIC 859 documents the staff and children’s files that were reviewed during this inspection.

Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in.

Menus are posted one month in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request.

First Aid supplies were observed in the classroom in a cabinet. According to the Program Specialist, medication is only administered to a child when accompanied with a doctor's note and is stored in each of the classroom's where it is inaccessible to children in care.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPA provided the Program Specialist with a copy of the Effects of Lead Exposure form during the inspection and discussed the following Provider Information Notices (PINs) Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD.

The following deficiencies listed on the attached deficiencies page are being cited in accordance with California Code of Regulations Title 22.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
